摘  要:目的通过分析结直肠癌患者术后胃肠功能恢复时间、术后住院时间、术后生活质量与I-FEED评分系统的关系,探讨其作为临床评价指标的有效性。方法选取2021年1—9月在汕头市某三级甲等医院胃肠外科参与临床研究的109例结直肠癌手术患者作为研究对象。采用病例报告表收集患者一般资料、肠鸣音恢复时间、首次排气时间、首次排便时间、术后住院时间等情况;采用欧洲五维健康量表(EQ-5D-3L)评估患者术后生存质量。按照I-FEED评分将患者分为术后胃肠功能恢复正常组55例、术后胃肠不耐受(POGI)组37例、术后胃肠功能障碍(POGD)组17例,采用单因素方差分析、卡方检验及非参数秩和检验分析3组间术后胃肠功能恢复时间、术后生活质量与I-FEED等级的关系。结果3组患者术后肠鸣音恢复时间、首次排气时间与术后住院时间比较差异有统计学意义(P<0.05),并且均以正常组患者的时间最短,其次是POGI组,而POGD组患者时间最长。3组患者术后1周生存质量5个维度均有所改善,并且正常组患者改善程度>POGI组>POGD组,其中行动能力、自理能力、疼痛或不适和焦虑或抑郁4个维度组间比较差异有统计学意义(P<0.05);而日常生活能力比较差异无统计学意义(P>0.05)。结论I-FEED评分系统作为结直肠癌术后患者胃肠功能状态的评价指标具有分层的敏感性和临床适用性。Objective To explore the effectiveness of the I-FEED scoring system as a clinical evaluation metric,this study analyzes its relationship with postoperative gastrointestinal recovery time,length of postoperative hospital stay,and postoperative quality of life in colorectal cancer patients.Methods A total of 109 patients who underwent colorectal cancer surgery and participated in a clinical study in the gastrointestinal surgery department of a tertiary grade A hospital in Shantou from January to September 2021 were selected as research subjects.The case report form was used to collect the patients'general information,bowel sound recovery time,time to first flatus,time to first bowel movement,length of postoperative hospital stay and other information.The European Quality of Life Five-Dimensional Scale(EQ-5D-3L)was used to assess the patients'postoperative quality of life.According to the I-FEED score,the patients were divided into three groups:a postoperative normal gastrointestinal function recovery group(n=55),a postoperative gastrointestinal intolerance(POGI)group(n=37),and a postoperative gastrointestinal dysfunction(POGD)group(n=17).One-way ANOVA,chi-square tests,and non-parametric rank-sum tests were used to analyze the relationships between postoperative gastrointestinal function recovery time,postoperative quality of life,and I-FEED grades among the three groups.Results There were statistically significant differences in postoperative bowel sound recovery time,time to first flatus and postoperative hospital stay among the three patient groups(P<0.05).The normal gastrointestinal function recovery group had the shortest times,followed by the POGI group,while the POGD group had the longest times.One week after surgery,all three patient groups showed improvements in the five dimensions of quality of life,with the degree of improvement being greatest in the normal group,followed by the POGI group,and the least in the POGD group.There were statistically significant differences in the four dimensions of mobility,s
