三联一体延续性护理模式对乳腺癌患者相关淋巴水肿的效果观察

摘　　要：目的探讨基于医院-社区-居家三联一体的延续护理模式对乳腺癌相关淋巴水肿患者综合消肿治疗效果的影响。方法选取某三甲医院乳腺外科2019年10月—2023年1月收治的60例淋巴水肿患者作为研究对象,均接受综合消肿治疗,根据入科时间分为对照组和观察组各30例。对照组接受院内健康指导+院外随访,观察组接受医院-社区-居家三联一体的延续护理模式干预,比较两组患者的上肢周径情况、淋巴水肿好转率、自觉症状体验、上肢功能及生活质量。结果观察组患者1、3个月的双侧上肢水肿周径差值、自觉症状体验评分和上肢功能评分均低于对照组,差异有统计学意义(P<0.05),观察组患者1、3个月的淋巴水肿好转率高于对照组,差异有统计学意义(P<0.05),生活质量评分经干预3个月后,观察组评分高于对照组,差异有统计学意义(P<0.05)。结论基于医院-社区-居家三联一体的延续护理模式干预可有效提高乳腺癌相关淋巴水肿患者的综合消肿疗法的效果,减轻淋巴水肿症状,改善上肢功能及生活质量。Objective To explore the effect of the hospital-community-home triple-integrated continuity nursing model on the comprehensive edema reduction treatment for patients with breast cancer-related lymphedema.Methods A total of 60 patients with lymphedema admitted to the department of breast surgery of a tertiary grade A hospital from October 2019 to January 2023 were selected as research subjects.All patients received comprehensive edema reduction treatment and were divided into a control group and an observation group based on their time of admission,with 30 cases in each group.The control group received in-hospital health guidance combined with out-of-hospital follow-up,while the observation group received intervention with the hospitalcommunity-home triple-integrated continuity nursing model.The upper limb circumference,improvement rate of lymphedema,subjective symptom experience,upper limb function and quality of life were compared between the two groups.Results The differences in bilateral upper limb edema circumference,subjective symptom experience scores,and upper limb function scores in the observation group were lower than those in the control group at 1 and 3 months,and the differences were statistically significant(P<0.05).The improvement rates of lymphedema in the observation group were higher than those in the control group at 1 and 3 months,and the differences were statistically significant(P<0.05).After 3 months of intervention,The quality of life score in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The hospital-community-home triple-integrated continuity nursing model intervention can effectively improve the effect of comprehensive detumescence therapy for patients with breast cancer-related lymphedema,alleviate lymphedema symptoms,and enhance upper limb function and quality of life.
