利多卡因乳膏联合硝酸甘油软膏在痔疮患者行外剥内扎术后镇痛中的临床研究

摘　　要：目的对比直肠肛管局部涂抹利多卡因乳膏联合硝酸甘油软膏在因痔疮行外剥内扎术后出现中度以上疼痛患者中的应用效果。方法回顾性选取2022年1月—2023年12月广西南宁市中医医院住院部收治的100例因痔疮行外剥内扎术患者的临床资料,根据治疗方式不同分为观察组和对照组,各50例。观察组采用直肠肛管局部涂抹利多卡因乳膏联合硝酸甘油软膏治疗,对照组口服镇痛药,对比两组术后疼痛水平、镇痛效果以及48 h给药频率。结果两组患者在术后6 h、24 h、36 h和48 h的疼痛评分对比,差异无统计学意义(P均>0.05)。术后观察组的疼痛缓解时间、48 h内的给药频率分别为(7.98±2.18)min和(6.06±1.95)次,低于对照组的(22.32±6.81)min和(7.58±1.15)次,差异有统计学意义(t=14.181、4.748,P均<0.05)。结论对因痔疮行外剥内扎术后患者进行疼痛干预时,采用直肠肛管局部涂抹利多卡因乳膏联合硝酸甘油软膏的局部镇痛效果更为高效。Objective To compare the application effect of local application of lidocaine cream combined with nitroglycerin ointment on rectal anal canal in patients with moderate or above pain after external dissection and internal ligation of hemorrhoids.Methods The clinical data of 100 patients with hemorrhoids who underwent external stripping and internal ligation in the inpatient department of Nanning Traditional Chinese Medicine Hospital from January 2022 to December 2023 were retrospectively selected and divided into observation group and control group according to different treatment methods,with 50 cases in each group.The observation group was treated with local application of lidocaine cream combined with nitroglycerin ointment,and the control group was treated with analgesics.The postoperative pain level,analgesic effect and 48 h administration frequency were compared between the two groups.Results There was no significant difference in pain scores between the two groups at 6 h,24 h,36 h and 48 h after operation(all P>0.05).The pain relief time and frequency of administration within 48 h in the observation group were(7.98±2.18)min and(6.06±1.95)times,respectively,which were lower than those in the control group(22.32±6.81)min and(7.58±1.15)times,the differences were statistically significant(t=14.181,4.748,both P<0.05).Conclusion In the pain intervention of patients after external dissection and internal ligation of hemorrhoids,the local analgesic effect of local application of lidocaine cream combined with nitroglycerin ointment in the rectum and anal canal is more efficient.
