宫腔镜手术联合地屈孕酮片治疗对子宫内膜息肉患者子宫内膜厚度、复发率及性激素的影响分析

摘　　要：目的分析宫腔镜手术联合地屈孕酮片治疗对子宫内膜息肉患者的临床效果。方法方便选取2022年1月—2023年12月扬州市江都妇幼保健院收治的108例子宫内膜息肉患者为研究对象,根据不同用药方案分为研究组和对照组,各54例。两组患者均行宫腔镜手术治疗,研究组术后口服地屈孕酮片,对照组口服黄体酮软胶囊。术前、术后1个月、术后3个月,对比两组患者的子宫内膜厚度、复发例数、雌孕激素[促黄体生成素、促卵泡激素、雌二醇]、炎症因子[白介素-13、白介素-6、超敏C反应蛋白]水平。结果术前,两组患者的子宫内膜厚度、雌孕激素水平、炎症因子水平对比,差异无统计学意义(P均>0.05);术后,研究组子宫内膜厚度、促黄体生成素水平、促卵泡激素水平、炎症因子水平均低于对照组,雌二醇高于对照组,差异有统计学意义(P均<0.05)。研究组复发率1.85%(1/54)低于对照组的14.81%(8/54),差异有统计学意义(χ^(2)=4.364,P<0.05)。结论地屈孕酮片治疗子宫内膜息肉可促进子宫内膜生长,降低复发率,改善雌孕激素水平及炎症因子水平。Objective To analyze the clinical effect of hysteroscopic surgery combined with dydrogesterone tablets on patients with endometrial polyps.Methods A total of 108 patients with endometrial polyps admitted to Yangzhou Jiangdu Maternal and Child Health Hospital from January 2022 to December 2023 were conveniently selected as the study objects and divided into study group and control group according to different medication regimens,with 54 cases in each group.Both groups were treated with hysteroscopic surgery.The study group was treated with oral dydrogesterone tablets after operation,and the control group was treated with progesterone soft capsules.Before operation,1 month after operation and 3 months after operation,the endometrial thickness,recurrence cases,sex hormone[luteinizing hormone,follicle stimulating hormone,estradiol],inflammatory factors(interleukin-13,interleukin-6,hypersensitive C-reactive protein)levels were compared between the two groups.Results Before surgery,there was no significant difference in endometrial thickness,estrogen and progesterone levels,and inflammatory factor levels between the two groups(all P>0.05).After surgery,the endometrial thickness,luteinizing hormone level,follicle stimulating hormone level,and inflammatory factor levels in the study group were lower than those in the control group,the estradiol level was higher than that in the control group,the differences were statistically significant(all P<0.05).The recurrence rate in the study group was 1.85%(1/54)lower than 14.81%(8/54)in the control group,the difference was statistically significant(χ^(2)=4.364,P<0.05).Conclusion Dydrogesterone tablets in the treatment of endometrial polyps can promote endometrial growth,reduce recurrence rate,improve estrogen and progesterone levels and inflammatory factor levels.
