经尿道双极等离子电切术治疗Ⅰ~Ⅱ期膀胱癌患者的临床疗效及安全性分析

摘　　要：目的分析经尿道双极等离子电切术治疗Ⅱ~Ⅲ期膀胱癌患者的效果及其安全性。方法便利选取2021年2月—2023年3月宁阳县第一人民医院收治的106例Ⅱ~Ⅲ期膀胱癌患者为研究对象,按治疗方法不同分为对照组、观察组,各53例。对照组予以膀胱部分切除术治疗,观察组行经尿道双极等离子电切术。比较两组治疗总有效率、肿瘤标志物水平及不良反应发生情况。结果观察组治疗总有效率为94.34%(50/53),高于对照组的81.13%(43/53),差异有统计学意义(χ^(2)=4.296,P<0.05)。治疗后,观察组细胞角蛋白19片段21-1、膀胱癌抗原1、胰岛素样生长因子-1、血管内皮生长因子水平均低于对照组,差异有统计学意义(P均<0.05)。观察组不良反应总发生率低于对照组,差异有统计学意义(P<0.05)。结论针对Ⅱ~Ⅲ期膀胱癌患者采取经尿道双极等离子电切术治疗效果显著,有利于改善患者的肿瘤标志物水平,且不良反应发生情况较少。Objective To analyze the efficacy and safety of transurethral bipolar plasma resection in patients with stageⅡ-Ⅲbladder cancer.Methods A total of 106 patients with stageⅡ-Ⅲbladder cancer treated in the First People's Hospital of Ningyang County from February 2021 to March 2023 were conveniently selected as the study objects,and were divided into control group and observation group according to different treatment methods,with 53 cases in each group.The control group was treated with partial cystectomy,and the observation group was treated with transurethral bipolar plasma resection.The total effective rate,tumor marker levels and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 94.34%(50/53),which was higher than 81.13%(43/53)of the control group,and the difference was statistically significant(χ^(2)=4.296,P<0.05).After treatment,the levels of cytokeratin 19 fragment 21-1,bladder cancer antigen 1,insulin-like growth factor-1 and vascular endothelial growth factor in observation group were lower than those in control group,and the differences were statistically significant(all P<0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Transurethral bipolar plasma resection is effective for patients with stageⅡ-Ⅲbladder cancer,which is beneficial to improve the level of tumor markers in patients,and the incidence of adverse reactions is less.
