早期应用无创呼吸机在急性左心衰急救中的效果评价

摘　　要：目的探究早期应用无创呼吸机在急性左心衰急救中的效果。方法方便选取2020年1月—2023年1月福建医科大学附属闽东医院收治的57例急性左心衰患者为研究对象,按照是否开展早期无创呼吸机治疗分为对照组[n=27,常规治疗动脉二氧化碳分压(partial pressure of carbon dioxide in arterial blood,PaCO_(2))≥45 mmHg开展无创机械通气]与研究组(n=30,PaCO_(2)<45 mmHg时实施无创呼吸机治疗),比较两组的心功能、动脉血气指标、心肌损伤因子水平、临床疗效及并发症发生情况。结果研究组的治疗总有效率为100.00%(30/30),高于对照组的74.07%(20/27),差异有统计学意义(χ^(2)=6.623,P<0.05)。研究组并发症发生率低于对照组,差异有统计学意义(P<0.05)。治疗后,研究组的左室射血分数、每搏量、动脉氧分压和血氧饱和度均优于对照组,差异有统计学意义(P均<0.05)。研究组的心肌损伤因子水平低于对照组,差异有统计学意义(P<0.05)。结论急性左心衰患者急救过程中早期应用无创呼吸机治疗,可提升治疗有效率,改善心功能,减少并发症的发生。Objective To explore the effect of early application of non-invasive ventilator in emergency treatment of acute left heart failure.Methods A total of 57 patients with acute left heart failure admitted to Fujian Medical University Affiliated Mindong Hospital from January 2020 to January 2023 were conveniently selected as the research objects and divided into control group[n=27,conventional treatment and non-invasive mechanical ventilation when partial pressure of carbon dioxide in arterial blood(PaCO_(2))≥45 mmHg]and study group(n=30,non-invasive ventilator treatment when PaCO_(2)<45 mmHg)according to whether early non-invasive ventilator treatment was carried out.The cardiac function,arterial blood gas index,the level of myocardial injury factors,clinical efficacy and complications were compared between the two groups.Results The total effective rate of treatment in the study group was 100.00%(30/30),which was higher than 74.07%(20/27)in the control group,and the difference was statistically significant(χ^(2)=6.623,P<0.05).The incidence of complications in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).After treatment,the left ventricular ejection fraction,stroke volume,arterial oxygen partial pressure and blood oxygen saturation of the study group were better than those of the control group,and the differences were statistically significant(all P<0.05).The level of myocardial injury factor of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Early application of non-invasive ventilator in the emergency treatment of patients with acute left heart failure can improve the treatment efficiency,improve cardiac function and reduce the occurrence of complications.
