摘 要:目的分析妊娠合并心脏病孕妇血清基质金属蛋白酶-1(MMP-1)和金属蛋白酶组织抑制物-1(TIMP-1)水平与妊娠结局的相关性。方法选取2019年8月—2021年9月在温州医科大学附属第一医院接受治疗的妊娠合并心脏病孕妇86例作为研究对象,依据妊娠结局分为妊娠结局良好组(61例)和妊娠结局不良组(25例)。采用妊娠风险分级量表评估孕妇的妊娠风险,采用酶联免疫吸附法测定两组孕妇血清MMP-1、TIMP-1水平。采用Spearman相关性分析法分析血清MMP-1、TIMP-1水平与妊娠风险分级及不良妊娠结局的相关性,采用受试者工作特征(ROC)曲线分析血清MMP-1、TIMP-1水平对不良妊娠结局的预测效能。结果妊娠风险分级Ⅰ~Ⅱ级孕妇的血清MMP-1、TIMP-1水平分别为(36.45±8.11)μg/L和(387.79±34.26)μg/L,妊娠风险分级Ⅲ~Ⅳ级孕妇的血清MMP-1、TIMP-1水平分别为(43.82±10.31)μg/L和(458.01±29.74)μg/L,妊娠风险分级Ⅲ~Ⅳ级孕妇的血清MMP-1、TIMP-1水平均显著高于妊娠风险分级Ⅰ~Ⅱ级的孕妇(t=3.711,9.951,均P<0.05)。妊娠结局不良组的血清MMP-1、TIMP-1水平明显高于妊娠结局良好组(均P<0.05)。相关性分析显示:血清MMP-1、TIMP-1水平与妊娠风险分级均呈正相关(r=0.511,P<0.05;r=0.487,P<0.05);血清MMP-1、TIMP-1水平与不良妊娠结局均呈正相关(r=0.458,P<0.05;r=0.496,P<0.05);妊娠风险分级与不良妊娠结局呈正相关(r=0.758,P<0.05)。血清MMP-1联合TIMP-1预测不良妊娠结局的曲线下面积(AUC)、灵敏度及特异度均高于单一指标检测。结论妊娠合并心脏病孕妇血清MMP-1、TIMP-1水平与不良妊娠结局相关,血清MMP-1和TIMP-1水平能够在一定程度上预测不良妊娠结局,二者联合检测对不良妊娠结局的预测效能更佳。Objective To analyze the correlations between serum levels of matrix metalloproteinase-1(MMP-1)and tissue inhibitor of metalloproteinase-1(TIMP-1)and pregnancy outcome in pregnant women with heart disease.Methods Eighty-six pregnant women complicated with heart disease treated in the First Affiliated Hospital of Wenzhou Medical University from August 2019 to September 2021 were selected as the study subjects.According to the pregnancy outcome,they were divided into good pregnancy outcome group(61 cases)and poor pregnancy outcome group(25 cases).The pregnancy risk of pregnant women was assessed by pregnancy risk grade scale,and the levels of serum MMP-1 and TIMP-1 were determined by enzyme-linked immunosorbent assay.The correlations between the levels of serum MMP-1 and TIMP-1,pregnancy risk grade,and adverse pregnancy outcome were analyzed by Spearman correlation analysis.Receiver operating characteristic(ROC)curve was used to analyze the predictive effect of serum MMP-1 and TIMP-1 levels for adverse pregnancy outcome.Results The levels of serum MMP-1 and TIMP-1 in pregnant women with pregnancy risk gradeⅠ-Ⅱwere(36.45±8.11)μg/L and(387.79±34.26)μg/L,respectively,the levels of serum MMP-1 and TIMP-1 in pregnant women with pregnancy risk gradeⅢ-Ⅳwere(43.82±10.31)μg/L and(458.01±29.74)μg/L,respectively,the levels of serum MMP-1 and TIMP-1 in pregnant women with pregnancy risk gradeⅢ-Ⅳwere significantly higher than those in pregnant women with pregnancy risk gradeⅠ-Ⅱ(t=3.711,9.951,P<0.05).The levels of serum MMP-1 and TIMP-1 in poor pregnancy outcome group were significantly higher than those in good pregnancy outcome group(P<0.05).Correlation analysis showed that serum MMP-1 and TIMP-1 levels were positively correlated with pregnancy risk grade(r=0.511,P<0.05;r=0.487,P<0.05),serum MMP-1 and TIMP-1 levels were positively correlated with adverse pregnancy outcome(r=0.458,P<0.05;r=0.496,P<0.05),pregnancy risk grade was positively correlated with adverse pregnancy outcome(r=0.758,P<0.05).The area
