摘　　要：目的观察左炔诺孕酮宫内节育系统联合促性腺激素释放激素激动剂(GnRH-α)治疗对子宫内膜异位症患者促卵泡激素和微炎症因子的影响。方法选取西安国际医学中心医院2020年10月至2023年10月收治的86例子宫内膜异位症患者,按照随机数字表法分为研究组和对照组,各43例,对照组采用GnRH-α治疗,研究组则联用左炔诺孕酮宫内节育系统,比较2组方案的临床疗效。结果研究组治疗总有效率高于对照组,且月经量评分、子宫内膜下血流阻力指数(RI)、搏动指数(PI),以及血管内皮生长因子(VEGF)、糖类抗原125(CA125)、促卵泡激素(FSH)、雌二醇(E2)、黄体生成素(LH)、单核细胞趋化蛋白-1(MCP-1)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)水平均低于对照组(P<0.05)。结论左炔诺孕酮宫内节育系统联合GnRH-α治疗子宫内膜异位症效果较好,能够有效调节患者性激素水平,抑制子宫内膜异常增殖,改善子宫血流动力学和微炎症刺激,从而缓解临床症状和预后表现,实用性较高。Objective To observe the effect of levonorgestrel combined with gonadotropin-releasing hormone agonist(GnRH)-αon follicle stimulating hormone and micro inflammatory factors in patients with endometriosis.Methods Eighty-six patients with endometriosis admitted to Xi′an International Medical Center Hospital from October 2020 to October 2023 were selected and randomly divided into a study group and a control group using a random number table method,with 43 cases in each group.The control group was treated with GnRH-αfor treatment,the research group used a combination of levonorgestrel to compare the clinical efficacy of the two groups.Results The total effective rate of treatment in the study group was higher than that in the control group,and menstrual flow score,subendometrial resistance index(RI),pulsatility index(PI),as well as vascular endothelial growth factor(VEGF),carbohydrate antigen 125(CA125),follicle stimulating hormone(FSH),estradiol(E2),luteinizing hormone(LH),monocyte chemotactic protein-1(MCP-1),interleukin-6(IL-6),tumor necrosis factor(TNF-α),and Interleukin-1β(IL-1)βin the study group were lower than those in the control group(P<0.05).Conclusion Levonorgestrel in collaboration with GnRH-αhas a good effect in the treatment of endometriosis,which can effectively regulate the patients′sex hormone levels,inhibit abnormal proliferation of the endometrium,improve uterine hemodynamics and micro inflammatory stimulation,thereby alleviating clinical symptoms and prognosis,and has high practicality.
