黄芪桂枝五物汤治疗气虚血瘀证糖尿病足临床观察

摘　　要：目的观察黄芪桂枝五物汤加味对气虚血瘀型糖尿病足的临床疗效。方法选取气虚血瘀型、Wagner分级为2~3级的糖尿病足患者96例,根据随机数字表法分为治疗组(46例)和对照组(47例),2组均接受降糖、抗感染、改善循环等常规西医治疗;治疗组在常规西医基础治疗上,加用黄芪桂枝五物汤加味。疗程3周。比较2组临床疗效、胰岛素样生长因子、白介素-6(IL-6)、踝肱指数(ABI)、溃疡面积的变化。结果治疗组临床总有效率高于对照组(P<0.05)。治疗后,治疗组创面完全愈合时间比对照组快(P<0.05);治疗组胰岛素样生长因子、ABI高于对照组,IL-6低于对照组(P<0.05)。结论黄芪桂枝五物汤加味治疗气虚血瘀型糖尿病足,在缓解临床症状、减轻炎症反应及改善供血情况方面有较好疗效。Objective To observe the clinical effect of Huangqi Guizhi Wuwu decoction on diabetic foot of Wagner grade 2~3 with qi deficiency and blood stasis syndrome.Methods A total of 96 patients with diabetic foot of Wagner grade 2~3 with qi deficiency and blood stasis syndrome were selected and divided into a treatment group(n=46)and a control group(n=47)according to the random number table method.Both groups received conventional Western medicine treatment such as hypoglycemic,anti-infection,and circulation improvement.The treatment group received Huangqi Guizhi Wuwu decoction on the basis of conventional Western medicine treatment for 3 weeks.The clinical efficacy,insulin-like growth factor,interleukin-6,ABI,and ulcer area were compared between the two groups.Results The total clinical effective rate of the treatment group was higher than that of the control group(P<0.05).The time of wound healing in the treatment group were faster than those in the control group(P<0.05).After treatment,the increase degree of insulin-like growth factor and ABI and the decrease degree of interleukin-6 in the treatment group were greater than those in the control group(P<0.05).Conclusion Huangqi Guizhi Wuwu decoction has a good effect in relieving clinical symptoms,reducing inflammatory response and improving blood supply in the treatment of diabetic foot of qi deficiency and blood stasis syndrome.
