从三焦论治湿热内蕴型干眼临证撷要

摘　　要：干眼是眼科门诊中比较常见的一种眼表疾病,病情复杂且缠绵难愈。三焦作为气和津液运行的通道,有运行水液、通行诸气的功能。湿热内蕴型干眼的病机为“湿热内蕴,三焦闭塞”,特点为各种原因引起的湿热之邪内蕴三焦,导致三焦气机不畅,进而津液输布异常,津不上承,目失濡润而发为此病,且伴有阴液的亏损。笔者基于多年临床经验总结出规律,并根据传统中医基础理论进行辨证加减,结合临床自拟利湿舒目汤,治则以清热祛湿、疏利三焦为主,同时兼顾滋养阴液。因此,文章对从三焦理论治疗湿热内蕴型干眼的经验进行了整理,并附上典型医案1则。Dry eye is a relatively common ocular surface disease in ophthalmology clinics,which is complex and difficult to be cured.As a channel between qi and body fluid,Sanjiao has the function of running water,liquid and qi.The pathogenesis of dry eye with dampness-heat connotation is“dampness-heat connotation and Sanjiao occlusion”,which is characterized by the dampness-heat evil connotation and Sanjiao occlusion caused by various reasons,resulting in the obstruction of the qi mechanism of the Sanjiao,and then the abnormal distribution of body fluid,the failure of fluid,the loss of eye moistening and resulting in this disease,and accompanied by the loss of yin fluid.Based on many years of clinical experience,the author summarized the law,and according to the basic theory of traditional Chinese medicine,the differentiation of syndromes,combined with clinical self-made Lishi Shumu decoction,the treatment is mainly to clear heat and dispel dampness,dredging Sanjiao,while taking into account nourishing yin fluid.Therefore,the experience of treating dampness-heat-insidious dry eye syndrome from the theory of Sanjiao was collated in this paper,and a typical medical case was attached.
