摘　　要：目的调查1例女性儿童发生金黄色葡萄球菌感染导致骨髓炎是否为注射新型冠状病毒灭活疫苗所引起。方法2022年2月1日—5月10日,县、乡两级专业人员开展个案调查,提取受种方、疫苗生产企业及接种方相关资料。2022年5月25日,菏泽市预防接种异常反应调查诊断专家组对该疑似预防接种异常反应(adverse events following immunization,AEFI)病例开展调查诊断,作出诊断意见。结果该儿童于2021年12月5日15点24分在某镇预防接种门诊在常规皮肤消毒后于其左上臂肌内注射0.5ml新型冠状病毒灭活疫苗(Vero细胞),观察30min无异常。其于2021年12月21日—2022年1月25日,以“发热、意识不清;头痛;左肱部肿痛”等为主诉,先后在三家医院住院治疗,出院诊断主要为骨髓炎、细菌性脑膜炎、脓毒血症(金黄色葡萄球菌)。预防接种过程符合操作规程,疫苗质量合格。市级调查诊断结论为偶合症。结论该女性儿童接种新型冠状病毒灭活疫苗与金黄色葡萄球菌感染发生骨髓炎无因果关系,判断为接种疫苗后的偶合症。Objective To investigate whether osteomyelitis caused by Staphylococcus aureus infection in a female child was caused by injection of inactivated SARS-CoV-2 vaccine.Methods From February 1 to May 10,2022,the county and township-level professionals carried out a case investigation and collected the relevant data of the vaccine recipient,the vaccine manufacturer and vaccinators.On May 25,2022,the expert group for the investigation and diagnosis of adverse events following immunization(AEFI)in Heze City conducted an investigation and diagnosis of the suspected AEFI case and made diagnostic opinions.Results The child was injected with 0.5ml of inactivated SARS-CoV-2 vaccine(Vero cells)into the muscle of her left upper arm after routine skin disinfection at 15:24 on December 5,2021 in a town vaccination clinic.No abnormalities were found after 30 minutes of observation.From December 21,2021 to January 25,2022,she was hospitalized in three hospitals successively with the main complaints of"fever and unconsciousness;headache;left humeral swelling pain".The discharge diagnosis was mainly:osteomyelitis;bacterial meningitis;pyemia(Staphylococcus aureus).The vaccination process was in line with the operating procedures and the vaccine quality was qualified.The conclusion of municipal investigation and diagnosis was:coupling disease.Conclusion There is no causal relationship between the inactivated SARS-CoV-2 vaccine administered and the osteomyelitis caused by Staphylococcus aureus infection in the female child,and it is judged to be a post-vaccination coupling disease.
