四川省名中医冷建春教授从“益气温阳”论治经皮冠状动脉支架植入术后再发心绞痛

摘　　要：经皮冠状动脉支架植入术后患者再发心绞痛的问题自冠脉支架植入术应用于临床之始便困扰患者及临床医师,本文整理并总结冷建春教授辨治经皮冠状动脉支架植入术后再发心绞痛的临证思路及用药经验。冷建春教授通过临床观察认为支架开通血管,为堵塞血管行活血化瘀之法,但忽略了患者心阳气亏虚,不能温煦血脉推动血行之本,因此相当比例的患者发生支架内再狭窄(in-stent restenosis,ISR),引起再发心绞痛、心肌梗死等不良心血管事件。他结合症状反复患者的证候特点,提出可以从“益气温阳”立法,随证选方,实现长期缓解患者症状的目标,同时为心血管疾病患者中西医结合治疗提供新方法和思路。This article summarizes Professor Leng Jianchun′s clinical thoughts and medication experience in treating recurrent angina pectoris after percutaneous coronary stent implantation:the problem of recurrent angina pectoris in patients after percutaneous coronary stent implantation has been troubling patients and clinical physicians since the introduction of coronary stent implantation into clinical practice.Professor Leng Jianchun,through clinical observations,believes that while stents open up blood vessels to promote Blood circulation and remove Blood stasis,they neglect the deficiency of Heart Yang and Qi in patients,leading to the inability to warm and promote Blood circulation.This can result in in-stent restenosis(ISR)within the stent,causing recurrent angina pectoris,myocardial infarction,and other adverse cardiovascular events.By repeatedly observing the characteristic symptoms of patients,he proposed a method of“benefiting Qi and warming Yang”based on individual conditions,aiming to achieve long-term relief of patients′symptoms.This approach provides a new method and direction for the integrated treatment of cardiovascular diseases using both traditional Chinese medicine and Western medicine.
