摘 要:目的对比分析对后交叉韧带胫骨附丽撕脱骨折患者实行关节镜与小切口手术的效果。方法回顾性选取2022年1—12月吉林医药学院附属医院收治的72例后交叉韧带胫骨附丽撕脱骨折患者的临床资料,根据不同治疗手段将其分成观察组(关节镜下双隧道缝线固定术,36例)、对照组(小切口切开复位内固定术,36例),比较两组的围手术期指标、关节活动度及活动水平、膝关节功能。结果观察组手术时间长于对照组,住院时间短于对照组,切口长度短于对照组,术中出血量少于对照组,差异有统计学意义(P均<0.05)。术前,两组患者的关节活动度、活动水平评分对比,差异无统计学意义(P均>0.05);术后6个月,观察组关节活动度、活动水平评分分别为(134.82±5.06)°和(8.31±1.08)分,均高于对照组的(131.98±5.18)°、(7.61±1.41)分,差异有统计学意义(t=2.353、2.365,P均<0.05)。术前,两组患者的膝关节功能评分对比,差异无统计学意义(P>0.05);术后6个月,观察组膝关节功能评分高于对照组,差异有统计学意义(P<0.05)。结论关节镜手术虽手术时间较长,但在术后康复情况上,恢复时间更短、功能状态恢复更快。Objective To compare and analyze the effect of arthroscopic and small incision surgery on patients with posterior cruciate ligament tibial appendage avulsion fracture.Methods The clinical data of 72 patients with appendage avulsion fracture of posterior cruciate ligament tibial in the Affiliated Hospital of Jilin Medical University from January to December 2022 were retrospectively analyzed.According to different treatment methods,they were divided into observation group(arthroscopic double tunnel suture fixation,36 cases)and control group(small incision open reduction and internal fixation,36 cases).Perioperative indexes,joint range of motion and activity level,and knee joint function were compared between the two groups.Results The operation time of the observation group was longer than that of the control group,the hospitalization time was shorter than that of the control group,the incision length was shorter than that of the control group,and the intraoperative blood loss was less than that of the control group,the differences were statistically significant(all P<0.05).Before operation,there was no significant difference in the scores of joint activity and activity level between the two groups(both P>0.05).At 6 months after operation,the scores of joint activity and activity level in the observation group were(134.82±5.06)°and(8.31±1.08)points,which were higher than(131.98±5.18)°and(7.61±1.41)points in the control group,and the differences were statistically significant(t=2.353,2.365,both P<0.05).Before operation,there was no significant difference in knee function score between the two groups(P>0.05).At 6 months after operation,the knee joint function score of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Although arthroscopic surgery has a longer operation time,it has a shorter recovery time and a faster recovery of functional status in postoperative rehabilitation.
