摘　　要：目的探讨临床上采用封闭式负压吸引技术(vacuum sealing drainage,VSD)治疗骨科创面感染对患者炎性因子、康复指标的影响。方法随机选取2019年1月—2023年1月连江县总医院(连江县医院)骨科收治的80例骨科创面感染患者为研究对象,根据不同治疗方法分为对照组和观察组,每组40例。对照组在进行彻底的清创处理后使用常规外科换药治疗,观察组在进行常规的清创处理后,采用VSD技术治疗。对比两组患者的超敏C-反应蛋白(hypersensitive C-reactive protein,hs-CRP)、白细胞介素-6(interleukin-6,IL-6)、红细胞沉降率(erythrocyte sedimentation rate,ESR)水平、换药次数、感染控制时间以及创面愈合时间。结果治疗后,两组的炎性因子水平均低于治疗前,且观察组的hs-CRP、IL-6、ESR分别为(3.11±1.01)mg/mL、(75.54±12.36)pg/mL、(13.56±4.63)mm/h,低于对照组的(6.21±1.26)mg/mL、(95.58±16.59)pg/mL、(19.54±3.98)mm/h,差异有统计学意义(t=12.141、6.126、6.195,P均<0.05)。观察组换药次数少于对照组,感染控制时间、创面愈合时间短于对照组,差异有统计学意义(P均<0.05)。结论VSD技术能有效减轻骨科创面感染患者的炎症反应,促进其康复进程,提高机体防御能力,有利于创面愈合。Objective To investigate the effects of clinical use of vacuum sealing drainage(VSD)in the treatment of orthopedic wound infections on patients'inflammatory factors and rehabilitation indexes.Methods A total of 80 patients with orthopedic wound infection admitted to the Department of Orthopedics,Lianjiang County General Hospital(Lianjiang County Hospital)from January 2019 to January 2023 were randomly selected as the research objects.According to different treatment methods,they were divided into control group and observation group,with 40 cases in each group.The control group was treated with routine surgical dressing change after thorough debridement,and the observation group was treated with VSD technology after routine debridement.The levels of hypersensitive C-reactive protein(hs-CRP),interleukin-6(IL-6),erythrocyte sedimentation rate(ESR),dressing change times,infection control time and wound healing time were compared between the two groups.Results After treatment,the levels of inflammatory factors in the two groups were lower than those before treatment,and the hs-CRP,IL-6 and ESR in the observation group were(3.11±1.01)mg/mL,(75.54±12.36)pg/mL and(13.56±4.63)mm/h,respectively,which were lower than(6.21±1.26)mg/mL,(95.58±16.59)pg/mL and(19.54±3.98)mm/h in the control group,the differences were statistically significant(t=12.141,6.126,6.195,all P<0.05).The number of dressing changes in the observation group was less than that in the control group,and the infection control time and wound healing time were shorter than those in the control group,the differences were statistically significant(all P<0.05).Conclusion VSD technology can effectively reduce the inflammatory response of patients with orthopedic wound infection,promote their recovery process,improve the body's defense ability,and facilitate wound healing.
