米库氯铵与苯磺顺阿曲库铵在老年患者气管插管中应用的对比研究

摘　　要：目的探讨米库氯铵与苯磺顺阿曲库铵在老年患者气管插管中的应用价值。方法单纯随机选取2023年1月—2024年2月福州市第一总医院收治的80例老年患者为研究对象,根据用药不同分为两组,各40例。术中气管插管时,对照组采用苯磺顺阿曲库铵,观察组采用米库氯铵,比较两组的心率(heart rate,HR)、平均动脉压(mean arterial pressure,MAP)的变化趋势、临床相关指标及药物不良反应。结果插管即刻,观察组的HR、MAP分别为(84.89±4.73)次/min、(93.52±3.49)mmHg,低于对照组的(87.13±4.57)次/min、(95.21±3.18)mmHg,插管后,观察组的HR、MAP分别为(79.54±5.21)次/min、(88.52±3.11)mmHg,高于对照组的(76.81±5.46)次/min、(87.06±3.27)mmHg,差异有统计学意义(t=2.154、2.264、2.288、2.046,P均<0.05)。观察组肌松药的起效时间、自主呼吸恢复时间、出复苏室时间均短于对照组,差异有统计学意义(P均<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论在老年手术患者气管插管中,米库氯铵具有较好的肌松效果,有利于维持术中HR、MAP指标的稳定,缩短术后自主呼吸恢复时间,苏醒质量良好。Objective To explore the application value of mivacurium chloride and cisatracurium besilate in tracheal intubation in elderly patients.Methods A total of 80 elderly patients admitted to Fuzhou First General Hospital from January 2023 to February 2024 were simply and randomly selected as the research objects,and they were divided into two groups according to different medication,with 40 cases in each group.During tracheal intubation,the control group was treated with cisatracurium besilate,and the observation group was treated with mivacurium chloride.The changes of heart rate(HR)and mean arterial pressure(MAP),clinical related indexes and adverse drug reactions were compared between the two groups.Results Immediately after intubation,the HR and MAP of the observation group were(84.89±4.73)times/min and(93.52±3.49)mmHg,which were lower than(87.13±4.57)times/min and(95.21±3.18)mmHg of the control group,after intubation,the HR and MAP of the observation group were(79.54±5.21)times/min and(88.52±3.11)mmHg,which were higher than(76.81±5.46)times/min and(87.06±3.27)mmHg of the control group,and the differences were statistically significant(t=2.154,2.264,2.288,2.046,all P<0.05).The onset time of muscle relaxants,the recovery time of spontaneous breathing and the time out of the resuscitation room in the observation group were shorter than those in the control group,and the differences were statistically significant(all P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Con⁃clusion In the tracheal intubation of elderly patients,mivacurium chloride has a good muscle relaxation effect,which is conducive to maintaining the stability of HR and MAP indexes during operation,shortening the recovery time of spontaneous breathing after operation,and improving the quality of recovery.
