米非司酮与米索前列醇联合治疗稽留流产的效果及胚胎自然排出率分析

摘　　要：目的探讨米非司酮与米索前列醇联合治疗稽留流产的效果。方法本研究方便选取2022年1月—2024年6月阳山县人民医院收治入院的96例稽留流产患者作为研究对象,根据不同用药方式分为两组,各48例。常规组患者仅给予米非司酮治疗,研究组患者则在米非司酮的基础上,联合使用米索前列醇治疗。对比两组患者血液学相关指标、临床相关指标、流产效果、胚胎自然排出率和不良反应。结果用药后,研究组的血液学相关指标优于常规组,阴道出血量少于常规组,腹痛持续、阴道出血、妊娠物排出及住院时间均短于常规组,差异有统计学意义(P均<0.05)。研究组的流产有效率及胚胎自然排出率分别为87.50%(42/48)与81.25%(39/48)均高于常规组的70.83%(34/48)与60.42%(29/48),差异有统计学意义(χ^(2)=4.042、5.042,P均<0.05)。研究组的不良反应发生率略高于常规组,但差异无统计学意义(P>0.05)。结论米非司酮与米索前列醇联合治疗稽留流产具有良好效果,能够提升流产有效率和胚胎自然排出率,同时减少阴道出血量,并缩短腹痛、阴道出血、妊娠物排出以及住院的时间。Objective To investigate the effect of combined treatment with mifepristone and misoprostol on missed abortion.Methods A total of 96 missed abortion patients admitted to Yangshan County People's Hospital from January 2022 to June 2024 were conveniently selected as the study objects.According to different medication methods,they were divided into two groups,with 48 cases in each group.Patients in the conventional group were only treated with mifepristone,while patients in the study group were treated with misoprostol on the basis of mifepristone.Hematological related indexes,clinical related indexes,abortion effect,natural embryo discharge rate and adverse reactions were compared between the two groups.Results After treatment,the hematology-related indexes of the study group were better than those of the conventional group,the vaginal bleeding volume was less than that of the conventional group,and at the same time,the duration of abdominal pain,vaginal bleeding,expulsion of pregnancy products and hospitalization time were shorter than those of the conventional group,the differences were statistically significant(all P<0.05).The abortion effective rate of 87.50%(42/48)and natural embryo excretion rate of 81.25%(39/48)in the study group were higher than those in the conventional group,which were 70.83%(34/48)and 60.42%(29/48),the differences were statistically significant(χ^(2)=4.042,5.042,both P<0.05).Although the incidence of adverse reactions was slightly higher in the study group than in the conventional group,but the difference was not statistically significant(P>0.05).Conclusion The combination of mifepristone and misoprostol in the treatment of missed abortion has a great effect,which can improve the abortion effective rate and natural embryo expulsion rate,reduce the amount of vaginal bleeding,shorten the time of abdominal pain,vaginal bleeding,pregnancy excretion and hospitalization.
