摘　　要：目的探究四肢长骨干骨折术后无菌性骨不连患者采用锁定钢板联合自体髂骨植骨术治疗的临床疗效。方法简单随机选取2022年1月—2023年12月山东省文登整骨烟台医院收治的80例四肢长骨干骨折术后无菌性骨不连患者作为研究对象,依据不同手术方法分为对照组和观察组,各40例。对照组实施交锁髓内钉联合侧方钢板内固定,观察组在对照组的基础上联合自体髂骨植骨术。对比两组临床指标、骨代谢水平、并发症发生率、治疗效果、Radius联合评分系统(Radius Union Scoring System,RUSS)评分。结果观察组术中出血量、术后引流量均少于对照组,内固定时间短于对照组,差异有统计学意义(P<0.05)。观察骨代谢水平、治疗优良率、RUSS评分高于对照组,差异有统计学意义(P均<0.05)。观察组并发症总发生率为2.50%(1/40),低于对照组的20.00%(8/40),差异有统计学意义(χ^(2)=4.507,P<0.05)。结论在对四肢长骨干骨折术后无菌性骨不连患者治疗时,锁定钢板联合自体髂骨植骨术可显著改善骨代谢水平,减轻疼痛,有助于加快骨愈合,且对并发症有较好的预防作用。Objective To explore the clinical efficacy of locking plate combined with autogenous iliac bone graft in the treatment of aseptic nonunion after long bone fracture of extremities.Methods From January 2022 to December 2023,80 patients with aseptic nonunion after long bone fracture of extremities admitted to Yantai Hospital of Shandong Wen⁃deng Orthopaedics&Traumatology were simply randomly selected as the research objects.According to different sur⁃gical methods,they were divided into control group and observation group,40 cases in each group.The control group was treated with interlocking intramedullary nail combined with lateral plate internal fixation,and the observation group was combined with autologous iliac bone grafting on the basis of the control group.The clinical indicators,bone metabolism level,complication rate,treatment effectiveness,and Radius Union Scoring System(RUSS)score were compared between the two groups.Results The intraoperative blood loss and postoperative drainage volume in the ob⁃servation group were less than those in the control group,and the internal fixation time was shorter than that in the control group,the differences were statistically significant(all P<0.05).The level of bone metabolism,excellent and good rate of treatment and RUSS score were higher than those in the control group,and the differences were statisti⁃cally significant(all P<0.05).The total incidence of complications in the observation group was 2.50%(1/40),which was lower than 20.00%(8/40)in the control group,and the difference was statistically significant(χ^(2)=4.507,P<0.05).Conclusion In the treatment of patients with aseptic nonunion after long bone fracture of extremities,locking plate combined with autogenous iliac bone grafting can significantly improve the level of bone metabolism,reduce pain,help to accelerate bone healing,and have a good preventive effect on complications.
