摘 要:目的研究尘肺病合并慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)治疗中应用体外式膈肌起搏器(external diaphragm pacer,EDP)的作用。方法便利选择2023年1月—2024年3月徐州矿务集团总医院收治的72例尘肺病合并COPD患者为研究对象,根据不同治疗方法分为对照组(n=36,内科常规治疗)、观察组(n=36,内科常规+EDP治疗)。比较两组的肺功能、血气指标、生活质量。结果治疗前,两组血气指标、肺功能指标、生活质量评分比较,差异无统计学意义(P均>0.05);治疗后,观察组第1 s用力呼气量占用力肺活量百分比、第1 s用力呼气量、最大通气量、血氧分压/吸入氧浓度、血氧分压均高于对照组,血二氧化碳分压低于对照组,差异有统计学意义(P均<0.05)。观察组的呼吸症状、活动受限、疾病影响评分分别为(53.11±5.26)分、(43.18±5.44)分、(39.51±3.45)分,低于对照组的(57.56±5.42)分、(49.35±6.12)分、(44.86±3.68)分,差异有统计学意义(t=3.535、4.521、6.364,P均<0.05)。结论尘肺病合并COPD行EDP治疗可改善患者肺功能与血气指标,提升生活质量。Objective To study the effect of external diaphragm pacemaker(EDP)in the treatment of pneumoconiosis complicated with chronic obstructive pulmonary disease(COPD).Methods A total of 72 patients with pneumoconiosis complicated with COPD admitted to Xuzhou Mining Group General Hospital from January 2023 to March 2024 were conveniently selected as the study subjects.They were divided into two groups according to different treatment meth⁃ods.The control group(n=36,conventional medical treatment),observation group(n=36,internal medicine routine+EDP treatment).The lung function,blood gas index and quality of life were compared between the two groups.Results Before treatment,there was no significant difference in blood gas index,lung function index and quality of life score between the two groups(all P>0.05);after treatment,the percentage of forced expiratory volume in the first second to forced vital capacity,forced expiratory volume in the first second,maximum ventilation volume,partial pressure of blood oxygen/fraction of inspiration O_(2) and partial pressure of blood oxygen in the observation group were higher than those in the control group,and the partial pressure of blood carbon dioxide was lower than that in the control group,the differences were statistically significant(all P<0.05).The scores of respiratory symptoms,activity limitation and disease impact in the observation group were(53.11±5.26)points,(43.18±5.44)points and(39.51±3.45)points,re⁃spectively,which were lower than(57.56±5.42)points,(49.35±6.12)points and(44.86±3.68)points in the control group,the differences were statistically significant(t=3.535,4.521,6.364,all P<0.05).Conclusion EDP treatment for pneumoconiosis combined with COPD can improve the lung function and blood gas index of patients and improve the quality of life.
