耳内镜下鼓膜置管术与鼓膜穿刺术治疗慢性分泌性中耳炎的临床疗效比较

摘　　要：目的 对比分析耳内镜下鼓膜置管术以及鼓膜穿刺术针对慢性分泌性中耳炎患者的临床治疗效果。方法 方便选取2021年1月—2023年9月南京医科大学第二附属医院收治的66例慢性分泌性中耳炎患者为研究对象,依据不同治疗方法分为对照组和观察组,每组33例。对照组患者接受鼓膜穿刺术治疗,观察组患者接受耳内镜下鼓膜置管术治疗。对比两组患者治疗效果、听力水平、临床时间指标、并发症发生情况。结果 观察组治疗总有效率为96.97%(32/33)高于对照组的75.76%(25/33),差异有统计学意义(χ^(2)=4.631,P<0.05)。治疗后,观察组骨导听力阈值、气导听力阈值均低于对照组,差异有统计学意义(P均<0.05)。观察组中耳积液时间及完全愈合时间均短于对照组,差异有统计学意义(P均<0.05)。观察组并发症总发生率为3.03%(1/33)低于对照组的27.27%(9/33),差异有统计学意义(χ^(2)=5.775,P<0.05)。结论 针对慢性分泌性中耳炎患者采用耳内镜下鼓膜置管术治疗,有助于提高疗效,改善听力,缩短中耳积液以及完全愈合时间,安全性较高。Objective To compare and analyze the clinical effects of endoscopic tympanic catheterization and tympa‐nocentesis on patients with chronic secretory otitis media.Methods 66 patients with chronic secretory otitis media ad‐mitted to the Second Affiliated Hospital of Nanjing Medical University from January 2021 to September 2023 were conveniently selected as the study objects,and were divided into control group and observation group according to dif‐ferent treatment methods,with 33 cases in each group.The control group received tympanocentesis,and the observa‐tion group received endoscopic tympanic catheterization.The therapeutic effect,hearing level,clinical time index and occurrence of complications of the two groups were compared.Results The total effective rate of the observation group was 96.97%(32/33)higher than that of the control group 75.76%(25/33),the difference was statistically significant(χ^(2)=4.631,P<0.05).After treatment,the bone conduction hearing threshold and air conduction hearing threshold in the observation group were lower than those in the control group,and the differences were statistically significant(both P<0.05).The time of middle ear effusion and complete healing in the observation group were shorter than those in the control group,and the differences were statistically significant(both P<0.05).The total incidence of complications in the observation group was 3.03%(1/33)lower than that in the control group 27.27%(9/33),and the difference was sta‐tistically significant(χ^(2)=5.775,P<0.05).Conclusion Endoscopic tympanic catheterization for patients with chronic se‐cretory otitis media is helpful to improve the efficacy,improve hearing,shorten the middle ear effusion and complete healing time,with high safety.
