自拟平肝益肾熄风方联合卡托普利治疗原发性高血压肝阳上亢证的临床效果

摘　　要：目的分析自拟平肝益肾熄风方联合卡托普利治疗原发性高血压肝阳上亢证的效果。方法选取2020年10月—2023年10月铜川市耀州区孙思邈中医院收治的160例原发性高血压肝阳上亢证患者为研究对象,以不同的治疗方法分为研究组(80例,接受自拟平肝益肾熄风方联合卡托普利治疗)和对照组(80例,接受卡托普利治疗),对比两组中医证候积分(眩晕、头痛、急躁易怒)、血压指标(24 h收缩压、24 h舒张压、24 h收缩压差、24 h舒张压差)、血清血管内皮细胞功能指标(内皮素1、一氧化氮)、不良反应(疲倦、咳嗽、失眠多梦)发生率。结果治疗后,研究组中医证候积分和血压指标低于对照组,差异有统计学意义(P均<0.05)。治疗后,研究组血管内皮素1水平为(75.22±10.33)ng/L,低于对照组的(79.98±11.21)ng/L,研究组一氧化氮水平为(56.74±8.88)mmol/L,高于对照组的(51.29±8.23)mmol/L,差异有统计学意义(t=2.793、4.026,P均<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论原发性高血压肝阳上亢证患者采取自拟平肝益肾熄风方联合卡托普利治疗可以更好地控制血压,安全性高。Objective To analyze the effect of self formulated Pinggan Yishen Xifeng Formula combined with captopril in the treatment of primary hypertension with liver yang hyperactivity syndrome.Methods A total of 160 patients with primary hypertension of liver yang hyperactivity syndrome admitted to Yaozhou District Sun Simiao Traditional Chinese Medicine Hospital of Tongchuan City from October 2020 to October 2023 were selected as the research objects.According to different treatment methods,they were divided into study group(80 cases,treated with self formulated Pinggan Yishen Xifeng Formula combined with captopril)and control group(80 cases,treated with captopril).The scores of Traditional Chinese Medicine(TCM)syndromes(vertigo,headache,irritability),blood pressure indexes(24 h systolic blood pressure,24 h diastolic blood pressure,24 h systolic blood pressure variability,24 h diastolic blood pressure variability),serum vascular endothelial cell function indexes(endothelin 1,nitric oxide),and incidence of adverse reactions(fatigue,cough,insomnia and dreaminess)were compared between the two groups.Results After treatment,the scores of TCM syndromes and blood pressure indexes in the study group were lower than those in the control group,and the differences were statistically significant(all P<0.05).After treatment,the level of vascular endothelin 1 in the study group was(75.22±10.33)ng/L,which was lower than(79.98±11.21)ng/L in the control group,the level of nitric oxide in the study group was(56.74±8.88)mmol/L,which was higher than(51.29±8.23)mmol/L in the control group,and the differences were statistically significant(t=2.793,4.026,both P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Self formulated Pinggan Yishen Xifeng Formula combined with captopril can better control blood pressure and has high safety in patients with primary hypertension with liver yang hyperactivity syndrome.
