中药灌肠配合穴位敷贴治疗脾肾气虚型慢性肾衰竭的临床研究

摘　　要：目的探讨中药灌肠配合穴位敷贴治疗脾肾气虚型慢性肾衰竭的临床效果。方法选取2021年8月—2023年6月上海市松江区方塔中医医院收治的80例脾肾气虚型慢性肾衰竭患者为研究对象,按治疗方法的不同将患者分为两组,每组40例。两组均接受常规治疗,同时,对照组辅以中药内服,观察组给予中药灌肠方灌肠+穴位敷贴治疗。对比两组肾功能水平、中医证候积分、治疗效果、不良反应发生情况。结果治疗后,观察组血肌酐、肾小球滤过率水平优于对照组,差异有统计学意义(P均<0.05)。治疗后,观察组中医证候积分为(10.15±2.89)分,低于对照组的(14.25±2.86)分,差异有统计学意义(t=6.378,P<0.05)。观察组治疗效果优于对照组,差异有统计学意义(P<0.05)。观察组不良反应总发生率为7.50%(3/40),低于对照组的15.00%(6/40),但差异无统计学意义(χ^(2)=0.501,P>0.05)。结论中药灌肠配合穴位敷贴治疗在改善脾肾气虚型慢性肾衰竭患者的肾功能和中医证候积分方面效果显著,且治疗效果优于传统的中药内服疗法,两者安全性相当。Objective To explore the clinical effect of traditional Chinese medicine enema combined with acupoint application in the treatment of chronic renal failure of spleen and kidney qi deficiency type.Methods From August 2021 to June 2023,80 patients with chronic renal failure of spleen and kidney qi deficiency type admitted to Shanghai Songjiang District Fangta Hospital of Traditional Chinese Medicine were selected as the study objects and divided into two groups according to different treatment methods,40 cases in each group.Both groups received routine treatment.At the same time,the control group was supplemented with oral administration of traditional Chinese medicine,the observation group was treated with traditional Chinese medicine enema+acupoint application.The renal function levels,traditional Chinese medicine syndrome score,therapeutic effect and adverse reactions were compared between the two groups.Results After treatment,the levels of serum creatinine and estimated glomerular filtration rate in observation group was better than those in the control group,and the differences were statistically significant(both P<0.05).After treatment,the traditional Chinese medicine syndrome score of the observation group was(10.15±2.89)points,which was lower than(14.25±2.86)points of the control group,and the difference was statistically significant(t=6.378,P<0.05).After treatment,the therapeutic effect of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).The total incidence of adverse reactions in the observation group was 7.50%(3/40),which was lower than 15.00%(6/40)in the control group,but the difference was not statistically significant(χ^(2)=0.501,P>0.05).Conclusion Traditional Chinese medicine enema combined with acupoint application has a significant effect on improving renal function and traditional Chinese medicine symptom scores in patients with chronic renal failure of spleen and kidney qi deficiency type,and the therapeutic effect was be
