摘  要:目的分析在重症肺炎并发呼吸衰竭患者中采取有创-无创序贯机械通气治疗的效果。方法选取2022年1月—2023年1月江苏省泗洪分金亭医院收治的112例重症肺炎并发呼吸衰竭患者为研究对象,按照不同治疗方法分为两组,各56例。对照组采取有创机械通气治疗,观察组采取有创-无创序贯机械通气治疗。比较两组呼吸机相关性肺炎及再插管发生率、动脉血二氧化碳分压、动脉血氧饱和度、动脉血氧分压、用力呼气量、第1秒用力呼气量水平。结果观察组呼吸机相关性肺炎发生率为7.14%(4/56)、再插管发生率为5.36%(3/56),均低于对照组的21.43%(12/56)、17.86%(10/56),差异有统计学意义(χ^(2)=4.667、4.264,P均<0.05)。观察组的血气分析指标(动脉血二氧化碳分压、动脉血氧饱和度、动脉血氧分压)均优于对照组,差异有统计学意义(P均<0.05)。观察组肺功能(用力呼气量、第1秒用力呼气量)均高于对照组,差异有统计学意义(P均<0.05)。结论重症肺炎并发呼吸衰竭患者采取有创-无创序贯机械通气治疗可改善血气分析指标及肺功能,具有一定的临床意义。Objective To analyze the effect of invasive-noninvasive sequential mechanical ventilation in patients with severe pneumonia complicated with respiratory failure.Methods A total of 112 patients with severe pneumonia complicated with respiratory failure admitted to Jiangsu Sihong Branch Jinting Hospital from January 2022 to January 2023 were selected as the research objects and divided into two groups according to different treatment methods,with 56 cases in each group.The control group was treated with invasive mechanical ventilation,and the observation group was treated with invasive-noninvasive sequential mechanical ventilation.The incidence of ventilator-associated pneumonia and reintubation,arterial carbon dioxide partial pressure,arterial oxygen saturation,arterial oxygen partial pressure,forced expiratory volume,and forced expiratory volume in the first second were compared between the two groups.Results The incidence of ventilator-associated pneumonia was 7.14%(4/56)and the incidence of the reintubation was 5.36%(3/56)in the observation group,which were lower than 21.43%(12/56)and 17.86%(10/56)in the control group,and the differences were statistically significant(χ^(2)=4.667,4.264,both P<0.05).The blood gas analysis indexes(arterial partial pressure of carbon dioxide,arterial oxygen saturation,arterial partial pressure of oxygen)in the observation group were better than those in the control group,and the differences were statistically significant(all P<0.05).The lung function(forced expiratory volume,forced expiratory volume in the first second)in the observation group were higher than that in the control group,and the differences were statistically significant(both P<0.05).Conclusion Invasive-noninvasive sequential mechanical ventilation can improve the blood gas analysis index and lung function of patients with severe pneumonia complicated with respiratory failure,which has certain clinical significance.
