磁共振胰胆管造影与多层螺旋CT扫描对胆总管结石的诊断价值对比分析

摘　　要：目的比较磁共振胰胆管造影(magnetic resonance cholangio-pancreatography,MRCP)与多层螺旋CT扫描(multi-slice spiral CT,MSCT)在胆总管结石诊断中的价值。方法选取2021年1月—2023年10月新沂市人民医院收治的106例疑似胆总管结石患者为研究对象。所有研究对象入院后均采取MRCP、MSCT及内镜逆行胰胆管造影(endoscopic retrograde cholangio-pancreatography,ERCP)检查。以ERCP检查结果为金标准,分析MRCP、MSCT在诊断胆总管结石中的准确度、特异度、灵敏度及对不同直径结石的诊断符合率。结果106例疑似胆总管结石患者中,ERCP检出阳性79例,阴性27例。MRCP诊断胆总管结石的准确度、特异度、灵敏度均高于MSCT,差异有统计学意义(P均<0.05)。MRCP对直径<0.5 cm、0.5 cm≤直径≤1.0 cm结石的诊断符合率高于MSCT,差异有统计学意义(P均<0.05)。在直径>1.0 cm的结石患者中,MSCT与MRCP的诊断符合率相比,差异无统计学意义(P>0.05)。结论在胆总管结石的早期诊断中,MRCP相对于MSCT更具优势,尤其对于直径较小的结石诊断的准确度更高。Objective To compared the value of magnetic resonance cholangio-pancreatography(MRCP)and multi slice spiral CT(MSCT)in the diagnosis of common bile duct stones.Methods A total of 106 suspected patients with common bile duct stones admitted to Xinyi People's Hospital from January 2021 to October 2023 were selected as the study subjects.All study subjects underwent MRCP,MSCT,and endoscopic retrograde cholangio-pancreatography(ERCP)examination after admission.Taking ERCP examination results as a reference gold standard,the accuracy,specificity,sensitivity,and diagnostic coincidence rate of MRCP and MSCT in diagnos of common bile duct stones were analyzed.Results Among 106 suspected patients with common bile duct stones,the ERCP check result is displayed 79 were positive and 27 were negative.The accuracy,specificity and sensitivity of MRCP in the diagnosis of common bile duct stones were higher than those of MSCT,and the differences were statistically significant(all P<0.05).The diagnostic coincidence rate of MRCP for stones with diameter<0.5 cm and 0.5 cm≤diameter≤1.0 cm was higher than that of MSCT,and the differences were statistically significant(both P<0.05).In patients with stones>1.0 cm in diameter,the diagnostic coincidence rate of MSCT and MRCP was compared,and the difference was not statistically significant(P>0.05).Conclusion In the early diagnosis of common bile duct stones,MRCP has more advantages than MSCT,especially for the diagnosis of small stones with higher accuracy.
