2型糖尿病合并糖尿病肾病的临床病理特征及预后分析

摘　　要：目的分析2型糖尿病(diabetes mellitus,DM)患者合并糖尿病肾病(diabetic kidney disease,DKD)的危险因素。方法回顾性选取2021年1月—2023年1月莆田市第一医院收治的124例糖尿病患者的临床资料,根据是否合并DKD将其分为单纯DM组75例,合并DKD组49例。分析两组的临床资料、实验室结果、病理检查结果,采用多因素回归方法评价DM合并DKD的影响因素。结果合并DKD组糖尿病病程长于单纯DM组,收缩压、血肌酐、低密度脂蛋白胆固醇、镜下血尿指标水平均高于单纯DM组,血红蛋白、肾小球滤过率指标水平均低于单纯DM组,差异有统计学意义(P均<0.05)。DM合并DKD患者预后的独立危险因素包括低密度脂蛋白胆固醇>3.5 mmol/L、镜下血尿≥3个(P均<0.05)。结论DM合并DKD患者预后的独立危险因素为低密度脂蛋白胆固醇>3.5 mmol/L、镜下血尿≥3个。Objective To analyze the risk factors of diabetic kidney disease(DKD)in patients with type 2 diabetes mellitus(DM).Methods The clinical data of 124 patients with diabetes admitted to the First Hospital of Putian City from January 2021 to January 2023 were retrospectively selected,and they were divided into DM group(75 cases)and DKD group(49 cases)according to whether they were combined with DKD.The clinical data,laboratory results and pathological findings of two groups were analyzed,and the influencing factors of DM with DKD were evaluated by multi-factor regression method.Results The course of diabetes in DKD group was longer than that in DM group,the systolic blood pressure,serum creatinine,low density lipoprotein cholesterol and microscopic hemuria indexes were higher than those in DM group,and the hemoglobin and glomerular filtration rate indexes were lower than those in DM group,the differences were statistically significant(all P<0.05).Independent risk factors for prognosis in DM patients with DKD included low density lipoprotein cholesterol>3.5 mmol/L and microscopic hematuria≥3(all P<0.05).Conclusion The independent risk factors for the prognosis of DM patients with DKD were low density lipoprotein cholesterol>3.5 mmol/L and microscopic hematuria≥3.
