摘  要:目的分析糖化血红蛋白、肾小球滤过率、尿β2-微球蛋白和空腹血糖在糖尿病肾病检测中的临床意义。方法选取2022年12月—2024年2月在泗阳医院收治的72例疑似糖尿病肾病患者为研究对象,均接受糖化血红蛋白、肾小球滤过率、尿β2-微球蛋白、空腹血糖检测,以临床诊断结果作为金标准。比较糖化血红蛋白、肾小球滤过率、尿β2-微球蛋白、空腹血糖检测以及联合检测的诊断效能。结果糖化血红蛋白、肾小球滤过率、尿β2-微球蛋白和空腹血糖4项指标联合检测的kappa值高于两项指标检测。糖化血红蛋白、肾小球滤过率、尿β2-微球蛋白和空腹血糖4项指标联合检测的灵敏度、准确度均高于两项指标检测,差异有统计学意义(P均<0.05)。结论糖化血红蛋白、肾小球滤过率、尿β2-微球蛋白和空腹血糖联合检测能够有效检出糖尿病肾病,且采用联合检测的方式比单一检测的准确度更高。Objective To analyze the clinical significance of glycated hemoglobin A1c,glomerular filtration rate,urinaryβ2-microglobulin and fasting plasma glucose in the detection of diabetic nephropathy.Methods A total of 72 patients with suspected diabetic nephropathy admitted to Siyang Hospital from December 2022 to February 2024 were selected as the research objects.All patients received glycated hemoglobin A1c,glomerular filtration rate,urinaryβ2-microglobulin and fasting plasma glucose detection.The clinical diagnosis results were used as the gold standard.The diagnostic efficacy of glycated hemoglobin A1c,glomerular filtration rate,urinaryβ2-microglobulin,fasting plasma glucose detection and combined detection were compared.Results The kappa value of combined detection of glycated hemoglobin A1c,glomerular filtration rate,urinaryβ2-microglobulin and fasting plasma glucose were higher than those of the two indicators.The sensitivity and accuracy of combined detection of glycated hemoglobin A1c,glomerular filtration rate,urinaryβ2-microglobulin and fasting plasma glucose were higher than those of the two indicators,and the differences were statistically significant(both P<0.05).Conclusion The combined detection of glycated hemoglobin A1c,glomerular filtration rate,urinaryβ2-microglobulin and fasting plasma glucose can effectively detect diabetic nephropathy,and the accuracy of combined detection is higher than that of single detection.
