糖化血红蛋白、胰岛素和D2聚体检测对筛查妊娠期糖尿病的价值分析

摘　　要：目的分析糖化血红蛋白、胰岛素、D2聚体联合检测用于临床筛查妊娠期糖尿病的价值。方法选取2022年6月—2024年5月句容市人民医院收治的80例初诊妊娠期糖尿病患者为观察组,同期接诊的80例健康妊娠者为对照组,均检测糖化血红蛋白、胰岛素及D2聚体水平。对比两组糖化血红蛋白、空腹胰岛素、餐后2 h胰岛素、D2聚体水平,分别计算单一指标检测与3项指标联合检测的灵敏度、特异度及准确度。结果观察组糖化血红蛋白、空腹胰岛素、餐后2 h胰岛素、D2聚体水平均高于对照组,差异有统计学意义(P均<0.05)。3项指标联合筛查的灵敏度、准确度均高于单一指标筛查,差异有统计学意义(P均<0.05)。结论通过糖化血红蛋白、胰岛素及D2聚体3项指标的检测可作为筛查妊娠期糖尿病的方法,操作简单,灵敏度及准确度均较高。Objective To analyze the value of combined detection of glycated hemoglobin A1c,insulin and D2 polymer in clinical screening of gestational diabetes mellitus.Methods A total of 80 patients with newly diagnosed gestational diabetes mellitus admitted to Jurong People Hospital from June 2022 to May 2024 were selected as the observation group,and 80 healthy pregnant women admitted during the same period were selected as the control group.The levels of glycated hemoglobin A1c,insulin and D2 polymer were detected.The levels of glycated hemoglobin A1c,fasting insulin,2-hour postprandial insulin and D2 polymer were compared between the two groups,and the sensitivity,specificity and accuracy of single index detection and combined detection of three indexes were calculated respectively.Results The levels of glycated hemoglobin A1c,fasting insulin,2-hour postprandial insulin and D2 polymer in the observation group were higher than those in the control group,and the differences were statistically significant(all P<0.05).The sensitivity and accuracy of the combined screening of the three indicators were higher than those of the single index screening,and the differences were statistically significant(both P<0.05).Conclusion The detection of glycated hemoglobin A1c,insulin and D2 polymer can be used as a method for screening gestational diabetes.The operation is simple,and the sensitivity and accuracy are high.
