二甲双胍和胰岛素治疗妊娠期糖尿病的临床效果及安全性

摘　　要：目的分析妊娠期糖尿病实行二甲双胍和胰岛素治疗的临床疗效。方法选取2022年11月—2023年12月鄄城马爱云医院收治的80例妊娠期糖尿病患者为研究对象,按照不同治疗方法分为两组,各40例,对照组单用胰岛素治疗,研究组采用二甲双胍联合胰岛素治疗,对比两组血糖指标、胰岛功能、并发症发生率、不良妊娠结局发生率。结果治疗后,研究组空腹血糖、餐后2 h血糖、糖化血红蛋白水平均低于对照组,差异有统计学意义(P均<0.05)。治疗后,研究组胰岛素及胰岛素抵抗指数均低于对照组,差异有统计学意义(P均<0.05)。研究组并发症发生率低于对照组,差异有统计学意义(P<0.05)。研究组不良妊娠结局发生率低于对照组,差异有统计学意义(P<0.05)。结论妊娠期糖尿病实行二甲双胍联合胰岛素治疗,可使患者血糖指标获得稳定,降低并发症发生率及不良妊娠结局发生率。Objective To analyze the clinical efficacy of metformin and insulin therapy for gestational diabetes mellitus(GDM).Methods A total of 80 patients with gestational diabetes mellitus admitted to Juancheng Maaiyun Hospital from November 2022 to December 2023 were selected as the research objects.According to different treatment methods,they were divided into two groups,with 40 cases in each group.The control group was treated with insulin alone,and the study group was treated with metformin combined with insulin.The blood glucose index,islet function,complication rate and adverse pregnancy outcome were compared between the two groups.Results After treatment,the fasting blood glucose,2-hour postprandial blood glucose and glycated hemoglobin A1c in the study group were lower than those in the control group,and the differences were statistically significant(all P<0.05).After treatment,the insulin and insulin resistance index in the study group were lower than those in the control group,and the differences were statistically significant(both P<0.05).The incidence of complications in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).The incidence of adverse pregnancy outcomes in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Metformin combined with insulin in the treatment of gestational diabetes mellitus can stabilize the blood glucose index of patients,reduce the incidence of complications and adverse pregnancy outcomes.
