摘  要:目的:探讨纤维蛋白原(Fib)、高密度脂蛋白(HDL)联合检测对创伤性下肢骨折术后深静脉血栓(DVT)的预测价值。方法:选取2020年12月—2022年12月天津市人民医院创伤骨科科室收治的75例创伤性下肢骨折术后患者作为研究对象,根据术后是否发生DVT将其分为DVT组(n=29)和非DVT组(n=46)。检测并比较两组患者血清Fib、HDL水平,采用ROC曲线分析血清Fib、HDL对创伤性下肢骨折术后DVT发生情况的预测价值。结果:DVT组Fib水平高于非DVT组,血清HDL水平低于非DVT组,差异有统计学意义(t=14.857、6.613,P<0.05);血清Fib、HDL二者联合受试者工作特征(ROC)曲线下面积(AUG)面积高于单独检测。结论:血清Fib、HDL水平与创伤性下肢骨折术后DVT发生情况关系密切,二者联合检测对早期诊断、治疗创伤性下肢骨折术后DVT意义重大。Objective:To investigate the predictive value of fibrinogen(Fib)and high-density lipoprotein(HDL)for deep vein thrombosis(DVT)after traumatic lower limb fracture.Methods:Selecting 75 postoperative patients of traumatic lower extremity fracture in Trauma Orthopedic Department from December 2020 to December 2022 and dividing them into DVT group(n=29)and non-DVT group(n=46).Serum Fib and HDL,and the predictive value of serum Fib and HDL on postoperative DVT of traumatic lower extremity fracture were analyzed.Results:The Fib level in the DVT group was higher than that in the non-DVT group,and the serum HDL level was lower than that in the non-DVT group,with statistically significant difference(t=14.857,6.613;P<0.05).The area under the combined ROC curve(AUG)of serum Fib and HDL was higher than that of single detection.Conclusion:The serum Fib and HDL levels are closely related to the occurrence of postoperative DVT after traumatic lower extremity fracture,and the combined detection of both patients is of great significance for the early diagnosis and treatment of postoperative DVT after traumatic lower extremity fracture.
