摘 要:目的:分析1型糖尿病患儿25羟维生素D水平、残存胰岛细胞功能和血糖控制情况。方法:选取2015年1月—2022年12月中国人民解放军陆军第八十三集团军医院收治的40例1型糖尿病患儿作为研究对象,将其设为观察组;另选取40例同期接受健康体检的儿童,将其设为对照组。检测比较两组患者血清25羟维生素D水平。1型糖尿病患儿接受维生素D口服治疗,对治疗前后的血清25羟维生素D水平、胰岛素使用剂量、糖化血红蛋白及C肽浓度进行观察比较。结果:观察组的血清25羟维生素D水平显著低于对照组,差异有统计学意义(t=4.4368,P<0.05);口服维生素D后,观察组的25羟维生素D、C肽水平均明显高于口服前(t=8.9610、12.8405,P<0.05),糖化血红蛋白水平明显低于口服前(t=6.8917,P<0.05)。结论:1型糖尿病患儿普遍表现出25羟维生素D缺乏,给予维生素D口服治疗不但能改善残存胰岛细胞功能,还能显著降低糖化血红蛋白水平,可有效控制血糖水平。Objective:To analyze the level of 25 hydroxyvitamin D,the function of residual islet cells and the effect of blood glucose control in children with type 1 diabetes.Methods:40 children with type 1 diabetes admitted to the 83rd Army Hospital of the PLA from January 2015 to December 2022 were selected as the study objects,and set as the observation group.Another 40 children who received physical examination during the same period were selected as the control group.Serum 25 hydroxyvitamin D levels were detected and compared between the two groups.The serum 25 hydroxyvitamin D level,insulin dose,glycosylated hemoglobin and C-peptide concentration before and after treatment were observed and compared in children with type 1 diabetes.Results:The serum 25-hydroxyvitamin D level in the observation group was significantly lower than that in the control group,and the difference was statistically significant(t=4.4368,P<0.05).After oral administration of vitamin D,the levels of 25-hydroxyvitamin D and C-peptide in the observation group were significantly higher than those before oral administration(t=8.9610,12.8405;P<0.05),and the level of glycated hemoglobin was significantly lower than that before oral administration(t=6.8917,P<0.05).Conclusion:Children with type 1 diabetes generally show 25 hydroxyvitamin D deficiency.Oral administration of vitamin D can not only significantly improve the function of residual islet cells,but also it can significantly reduce the level of glycosylated hemoglobin,and effectively control blood glucose level.
