游离桡动脉掌浅支带正中神经小皮瓣修复手部软组织缺损的疗效分析

摘　　要：目的:采用桡动脉掌浅支携带正中神经浅支游离皮瓣修复手指软组织缺损术后疗效。方法:回顾性分析2020年6月—2022年12月蚌埠医学院第一附属医院收治的25例手部软组织缺损患者的临床资料。所有患者均采取桡动脉掌浅支携带正中神经浅支游离皮瓣修复指腹缺损,均获得术后的随访,随访观察患者手部外形、手部功能、局部感觉。结果:25例患者均获得术后的随访。所有皮瓣均存活,1例患者,术后第2 d出现皮瓣的远端肿胀、起水泡,予以拆除缝线后,并出现部分坏死,未见好转,术后3~4周瘢痕褪去,愈合良好。所有患者手指外形美观,手部的屈伸功能未受影响,皮瓣质地柔软,手部皮瓣感觉恢复好。所有患指创面感觉恢复良好,皮瓣两点分辨觉可达到6~8 mm。指功能依据中华医学会手外科学会上肢部分功能评定试用标准进行评定,优24例,良1例。结论:使用桡动脉掌浅支携带正中神经浅支游离皮瓣能有效修复手部皮肤缺损,手部外形、美观、局部感觉及手部功能有效恢复。Objective:The postoperative effect of the superficial branch of radial artery to repair the soft tissue defect of fingers.Methods:A total of 25 patients with hand soft tissue defects were collected from June 2020 to December 2022.All patients took the superficial branch of the radial artery and the free flap to repair the abdominal defect.All postoperative follow-up was obtained to observe the hand shape,hand function and local sensation.Results:Post-operative follow-up was obtained in all 25 patients.All flaps were alive.In 1 patient,the suture was removed the following day,with partial necrosis and no improvement,and the scar faded 3~4 weeks after surgery and healed well.All the patients had beautiful finger appearance,the flexion and extension function of the hand was not affected,the flap was soft,and the hand flap felt good recovery.All affected finger wounds had good recovery,and the resolution of the flap could reach 6~8mm.The function was evaluated according to the trial standard of upper limb function evaluation of Chinese Hand Surgery Association,with 24 cases excellent and 1 good case.Conclusion:The free flap of the superficial branch of the radial artery carrying the superficial branch of the median nerve can effectively repair the skin defect of the hand,and effectively restore the hand appearance,artistry,local sensation and the function of the hand.
