血清RA33抗体、抗CCP抗体、RF及抗AKA联合检测对类风湿性关节炎的诊断价值分析

摘　　要：目的:分析血清RA33抗体、抗环瓜氨酸肽(CCP)抗体、类风湿因子(RF)及抗角蛋白抗体(AKA)联合检测对类风湿性关节炎(RA)的诊断价值。方法:选择2022年8月—2023年7月茂名市人民医院治疗的300例RA患者作为RA组;同时选取同期在医院治疗的非RA自身免疫性疾病患者280例作为非RA组。检测两组患者血清RA33抗体、抗CCP抗体、RF及抗AKA水平。比较两组患者RA33抗体、抗CCP抗体、RF及抗AKA阳性检出率;计算RA33抗体、抗CCP抗体、RF及抗AKA单项及联合诊断RA的灵敏度、特异度。结果:RA组RA33抗体、抗CCP抗体、RF及抗AKA阳性检出率高于非RA组,差异有统计学意义(χ^(2)=219.891、265.143、182.448、335.638,P<0.05);RA33抗体+抗CCP抗体+RF+抗AKA联合诊断RA的灵敏度最高为93.00%(279/300),抗CCP抗体+抗AKA联合诊断RA的灵敏度与特异度均较高,灵敏度为87.00%(261/300)、特异度为90.36%(253/280)。结论:血清RA33抗体、抗CCP抗体、RF及抗AKA联合检测能够提高RA诊断灵敏度,提高RA早期诊断效能,有利于降低RA漏诊率。Objective:To analyze the diagnostic value of combined detection of serum RA33 antibody,anti cyclic citrullinated peptide(CCP)antibody,rheumatoid factor(RF),and anti keratin antibody(AKA)in rheumatoid arthritis(RA).Methods:300 RA patients who were treated in the hospital from August 2022 to July 2023 were selected as the RA group.At the same time,280 patients with non RA autoimmune diseases who were treated in the hospital during the same period were selected as the non RA group.Detecting the levels of serum RA33 antibody,anti CCP antibody,RF and anti AKA in two groups;comparing the positive detection rates of RA33 antibody,anti CCP antibody,RF,and anti AKA between two groups;and calculating the diagnostic sensitivity and specificity of RA33 antibody,anti CCP antibody,RF and anti AKA single and combined diagnosis for RA.Results:The positive detection rates of RA33 antibody,anti CCP antibody,RF and anti AKA in the RA group were higher than those in the non RA group,with statistically significant difference(χ^(2)=219.891,265.143,182.448,335.638;P<0.05).The highest sensitivity of the combination of RA33 antibody,anti CCP antibody,RF and anti AKA in the diagnosis of RA was 93.00%(279/300).The sensitivity and specificity of the combination of anti CCP antibody and anti AKA in the diagnosis of RA ware both high,with a sensitivity of 87.00%(261/300)and a specificity of 90.36%(253/280).Conclusion:The combined detection of serum RA33 antibody,anti CCP antibody,RF and anti AKA can improve the diagnostic sensitivity and early diagnostic efficacy of RA,which is beneficial for reducing the missed diagnosis rate of RA.
