北京市丰台区高脂血症患者SLCO1B1与ApoE基因多态性分析

摘  要:目的:本研究旨在调查北京丰台地区高脂血症患者中SLCO1B1和载脂蛋白E(ApoE)基因的多态性分布情况。方法:选取2022年1—12月北京航天总医院神经内科住院需服用他汀类药物调控血脂的645例高脂血症及患有心脑血管疾病患者作为研究对象,其中男性354例,女性291例,患者年龄27~96岁。使用荧光定量聚合酶链式反应(PCR)技术对SLCO1B1和ApoE基因进行基因分型检测,然后统计不同基因型和代谢型的分布频率,分析不同性别和年龄患者对SLCO1B1与ApoE基因的分布的影响。结果:在645例标本中SLCO1B1基因多态性纯野生型(TT)、杂合性(TC)、纯突变型(CC)分布频率分别为74.7%、24%、1.3%;ApoE基因型E2/E2、E2/E3、E2/E4、E3/E3、E3/E4、E4/E4分布频率为0.1%、14.0%、0.8%、73.1%、10.9%、1.1%。年龄和性别对高脂血症患者SLCO1B1与ApoE基因型分布,差异无统计学意义(P>0.05)。结论:北京市丰台区高脂血症患者存在SLCO1B1与ApoE基因多态性分布不均,脂代谢异常可能与SLCO1B1和ApoE基因多态性有关。对于那些患有高脂血症和其他脂代谢异常的患者,服用他汀类药物前建议临床先开展SLCO1B1与ApoE基因检测,从而更好地实施个体化用药。Objective:To investigate the polymorphism distribution of SLCO1B1 and ApoE genes in hyperlipidemic patients in Fengtai Area of Beijing.Methods:645 patients with hyperlipidemia,and cardiovascular and cerebrovascular diseases who were admitted to the Department of Neurology in Beijing Aerospace General Hospital from January to December 2022 and required statins to regulate blood lipids were selected as the study subjects,including 354 males and 291 females,aged 27 to 96 years old.Using fluorescence PCR technology to genotyping SLCO1B1 and ApoE genes,and analyzing the distribution frequency of different genotypes and metabolic types,and the impact of different gender and age patients on the distribution of SLCO1B1 and ApoE genes.Results:The distribution frequencies of SLCO1B1 gene polymorphism TT(pure wild-type),TC(heterozygous)and CC(pure mutant)in 645 specimens were 74.7%,24.0%and 1.3%,respectively.The distribution frequencies of APOE genotypes E2/E2,E2/E3,E2/E4,E3/E3,E3/E4,and E4/E4 were 0.1%,14.0%,0.8%,73.1%,10.9%and 1.1%.The genotype distribution of SLCO1B1 and APOE in patients with hyperlipidemia was unaffected by age or gender statistically(P>0.05).Conclusion:Patients with hyperlipidemia in Fengtai District of Beijing have uneven distribution of SLCO1B1 and APOE gene polymorphisms,and abnormal lipid metabolism may be related to SLCO1B1 and ApoE gene polymorphisms.For patients with hyperlipidemia and other lipid metabolism abnormalities,it is recommended to conduct SLCO1B1 and ApoE gene testing in clinical practice before taking statins,in order to better implement personalized medication.
