纤维支气管镜辅助下经皮扩张气管切开术在中医院ICU的临床应用价值

摘　　要：目的:探讨纤维支气管镜辅助下经皮扩张气管切开术(percutaneous dilation tracheostomy,PDT)在中医院重症监护病房(Intensive Care Unit,ICU)的临床应用价值。方法:回顾性分析2018年1月—2023年12月昆明市中医医院重症医学科收治的48例行气管切开术患者的临床资料,根据手术方式将其分为传统外科手术气管切开组(传统手术组)和纤维支气管镜辅助下经皮扩张气管切开组(纤支镜组),各24例。比较两组手术切口、手术时间、术中出血量、切口缝合人数及术后并发症。结果:纤支镜组手术时间短于传统手术组,手术切口小于传统手术组,术中出血量少于传统手术组,切口缝合人数少于传统手术组,术后切口渗血、皮下气肿及导管位置偏移发生率低于传统手术组,差异有统计学意义(P<0.05);但两组术后切口感染发生率比较,差异无统计学意义(P>0.05)。结论:纤维支气管镜辅助下PDT具有安全、便捷、手术切口小、手术时间短、出血量少、术后并发症少等优点。Objective:To explore the clinical application value of percutaneous dilation tracheotomy(PDT)assisted by fiberoptic bronchoscopy in Intensive Care Unit(ICU)of traditional Chinese medicine hospital.Method:The clinical data of 48 patients who underwent tracheotomy in the Department of Critical Care Medicine of Kunming Municipal Hospital of Traditional Chinese Medicine from January 2018 to December 2023 were retrospectively analyzed,they were divided into traditional surgical tracheotomy group(traditional surgery group)and PDT assisted by fiberoptic bronchoscopy group(fiberoptic bronchoscopy group)according to the surgical methods,with 24 cases in each group.The surgical incision,operation time,intraoperative blood loss,number of incision sutures and postoperative complications were compared between two groups.Result:The operation time of the fiberoptic bronchoscopy group was shorter than that of the traditional surgery group,the surgical incision was smaller than that of the traditional surgery group,the intraoperative blood loss was less than that of the traditional surgery group,the number of incision sutures was less than that of the traditional surgery group,and the incidence of postoperative incision bleeding,subcutaneous emphysema and catheter position deviation were lower than those of the traditional surgery group,the differences were statistically significant(P<0.05);however,there was no significant difference in the incidence of postoperative incision infection between two groups(P>0.05).Conclusion:PDT assisted by fiberoptic bronchoscopy has the advantages of safety,convenience,small incision,short operation time,less bleeding and less postoperative complications.
