吴雄志教授从“少阴热化”论治更年期综合征临证探析

摘　　要：更年期综合征为临床常见病、多发病,高发于围绝经期女性。此病常见焦虑烦躁、失眠、心悸等症状。西医认为发病原因主要是卵巢功能衰退致内分泌紊乱、性激素水平波动(尤其指雌激素减少),常用激素代替疗法、镇静疗法等治疗。中医将此病归于“脏燥”“百合病”“郁证”范畴。中医流派众多,治疗思路各异,方法多样,但可复制性差。在“五法六经”理论指导下,吴雄志教授认为更年期综合征属于六经“少阴病”,核心病机为“少阴热化”。而“少阴热化”又分为“少阴心”“少阴肾”,更年期综合征以“少阴肾”为主。治疗时,吴雄志教授强调“直取其病,随症加减”,并开创吴门验方并济饮专治。研究从六经辨证、中西汇通角度探讨更年期综合征与“少阴热化”,结临证经验探析并济饮组方规律及其学术思想,以期拓展更年期综合征的中医治疗思路。Menopausal syndrome is a common and frequently occurring clinical disease,which is highly prevalent in perimenopausal women.This disease often has symptoms such as anxiety,irritability,insomnia,palpitations,etc.Western medicine believes that the main causes of its pathogenesis are endocrine disorders and sex hormone fluctuations(especially estrogen reduction)caused by ovarian function decline.Western medicine often uses hormone replacement therapy,sedation and other therapies to treat this disease.In traditional Chinese medicine,the disease is classified into hysteria,lily disease,depression syndrome and other categories.There are many schools of traditional Chinese medicine,with different treatment ideas and methods.Although each takes its effect,it is difficult to grasp its core,resulting in poor reproducibility.Under the guidance of the Five methods and Six meridians theory,Professor WU Xiongzhi believes that menopausal syndrome belongs to the Shaoyin disease(少阴病)of the Six meridians.Its core pathogenesis is Shaoyin Rehua(少阴热化).The Shaoyin Rehua is further divided into Shaoyin-heart(少阴心)and Shaoyin-kidney(少阴肾).The menopausal syndrome is classified as Shaoyin-kidney mainly.In terms of treatment,Professor WU Xiongzhi emphasized taking the disease directly and adjusting the medication according to the symptom,and created WU’s proven prescription Bingji Yin(并济饮)for the treatment of menopausal syndrome.This paper discusses the menopausal syndrome and Shaoyin Rehua from the perspectives of six meridians differentiation and the convergence of traditional Chinese medicine and Western medicine,and combines with the author's clinical experience to analyze the prescription rules and academic thoughts of Bingji Yin,in order to enrich and expand the traditional Chinese medicine treatment ideas of menopausal syndrome.
