益气活血利水方对气虚血瘀型慢性心力衰竭患者的疗效及对炎症因子的影响研究

摘　　要：目的观察益气活血利水方治疗气虚血瘀型慢性心力衰竭(CHF)患者的临床效果,并探讨此方对患者体内炎症因子的影响,以了解益气活血利水方治疗气虚血瘀型CHF患者的作用机制,为该方进一步开发利用提供科学的试验依据。方法选取2019年12月—2020年12月在本院就诊的60例气虚血瘀型CHF患者为研究对象,根据随机数字表法平均分为研究组和对照组,每组30例。其中,对照组仅给予西药常规治疗,而研究组在给予对照组同等治疗的基础上配用益气活血利水方,疗程均为14d。比较两组治疗前后临床症状总积分、有效率、B脑钠肽(BNP)、C反应蛋白(CRP)及同型半胱氨酸(Hcy)的差异。结果治疗后,研究组治疗有效率高于对照组,差异有统计学意义(P<0.05)。研究组BNP、临床症状总积分、CRP及Hcy改善程度优于对照组,差异有统计学意义(P<0.05)。两组治疗过程中谷丙、门冬氨酸转移酶、肌酐及尿素氮等也未见明显变化(P>0.05)。结论在治疗确诊为气虚血瘀型CHF患者时,采用加用益气活血利水方的治疗方案,能够显著地降低患者的炎症指标,进而改善其临床症状,并有效提升患者的心功能水平。Objective To observe clinical effect of replenishing qi,activating blood and freeing urination formula for patients with CHF and Qi deficiency and blood stasis type,and explore its effect for inflammatory factors of patient,to understand mechanism of action of replenishing qi,activating blood and freeing urination formula for patients with chronic heart failure and Qi deficiency and blood stasis type,and provide scientific experimental basis for further development and utilization of the formula.Methods The paper chose 60 patients with chronic heart failure of Qi deficiency and blood stasis type in cardiology department of Qinghai Provincial Hospital of Traditional Chinese Medicine from December 2019 to December 2020,and divided them into study group and control group evenly with random number table method,with 30 cases in each group.During the study,control group was treated with routine Western medicine treatment,while study group with the same treatment as control group with combination of replenishing qi,activating blood and freeing urination formula.Treatment period lasted for 14 days.Difference in total clinical symptom scores,effective rate,B-brain natriuretic peptide(BNP),C-reactive protein(CRP),and homocysteine(Hcy)was compared between two groups before and after treatment.Results After treatment,the treatment effective rate of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).The improvement of CRP and Hcy in the total score of BNP clinical symptoms in the study group was better than that of the control group,and the difference was statistically significant(P<0.05)Creatinine and urea nitrogen were not significantly increased(P>0.05).Conclusion Replenishing qi,activating blood and freeing urination formula can reduce inflammatory indicators,improve clinical symptoms and enhance heart function of chronic heart failure patients patients with Qi deficiency and blood stasis effectively.
