利用吉尼环固定曼月乐环治疗子宫体积过大的子宫腺肌病疗效分析

摘　　要：目的研究吉尼环固定曼月乐环治疗子宫体积过大子宫腺肌症患者的疗效。方法选取2022年1月—2023年7月本院收治的60例子宫体积过大子宫腺肌症患者为研究对象,并按照随机抓阄法分为对照组(30例)和观察组(30例)。其中,对照组采用GnRH-a类药物治疗,观察组采用吉尼环固定曼月乐环治疗。对比两组患者的临床治疗效果、激素水平指标、血清学指标、子宫内膜厚度和月经量、痛经评分、性生活质量评分、不良反应情况。结果观察组总有效率高于对照组(P<0.05)。观察组患者促黄体生成素、卵泡刺激素、雌二醇低于对照组(P<0.05)。观察组患者血管内皮生长因子、基质金属蛋白酶9低于对照组(P<0.05)。观察组患者子宫内膜厚度和月经量低于对照组(P<0.05)。观察组患者VAS痛经评分低于对照组、FSFI性生活质量评分高于对照组(P<0.05)。观察组潮热、失眠、阴道干燥、性欲减退不良反应发生率低于对照组(P<0.05);骨质疏松不良反应发生率略低于对照组,差异无统计学意义(P>0.05)。结论吉尼环固定曼月乐环治疗子宫体积过大子宫腺肌症患者的疗效显著优于GnRH-a类药物治疗,因此吉尼环固定曼月乐环治疗更具推广价值。Objective To study curative effect of Gyne ring fixed Mirena ring in treatment of adenomyosis patients with metrauxe metrypertrophia.Methods The paper chose 60 adenomyosis patients with metrauxe metrypertrophia in our hospital from January 2022 to July 2023,and divided them into control group(30 cases)and observation group(30 cases)randomly by drawing lots.Control group was treated with GnRH-a drugs,while observation group with Gyne ring fixed Mirena ring.Clinical curative effect,hormone level indicators,serological indicators,endometrial thickness and menstrual volume,dysmenorrhea scores,sexual life quality scores and adverse reactions were compared between two groups.Results Total effective rate of observation group was higher than control group,difference in comparison results was significant(P<0.05).Levels of luteinizing hormone,follicle stimulating hormone,and estradiol in observation group(treated with Gyne ring fixed Mirena ring)were lower than control group,difference in comparison results was significant,(P<0.05).Levels of vascular endothelial growth factor and matrix metalloproteinase-9 in observation group(treated with Gyne ring fixed Mirena ring)were lower than control group(with GnRH-a drugs),difference in comparison results was significant(P<0.05).Observation group(treated with Gyne ring fixed Mirena ring)had significantly lower endometrial thickness and menstrual flow than control group(with GnRH-a drugs),difference in comparison results was significant(P<0.05).VAS dysmenorrhea scores of observation group was lower than control group,and FSFI sexual life quality scores were higher than control group,difference in comparison results was significant(P<0.05).The incidence of adverse reactions such as hot flashes,insomnia,vaginal dryness,and decreased libido in the observation group was lower than that in the control group(P<0.05);The incidence of adverse reactions in osteoporosis was slightly lower than that in the control group,and the difference was not significant(P>0.05).Conclusion Gyne ring fixed
