针刺治疗慢性阻塞性肺疾病稳定期选穴规律的研究

摘　　要：目的探索针刺治疗慢性阻塞性肺疾病(COPD)稳定期的临床选穴特点和规律。方法检索数据库,按纳入、排除标准筛选文献,提取针刺处方及腧穴归经等信息建立数据库,采用Apriori关联规则分析、系统聚类分析等数据挖掘方法对针刺选穴进行频次、归经、分布及关联规则、聚类分析。结果合计纳入文献63篇,提炼针刺处方55条,涉及腧穴53个,累计频次406次。使用频率排名前3的腧穴依次为肺俞、膻中、足三里;所属经脉主要有足太阳膀胱经、足阳明胃经任脉和手太阴肺经;选穴部位多涉及胸腹、腰背部;特定穴选用以背俞穴、五输穴、募穴、八会穴为多。关联规则分析得强关联规则10条,如肾俞、肺俞、定喘+足三里、肺俞、肾俞+足三里、肺俞等;聚类分析得到4个聚类群,分别为:①肺俞、定喘、足三里;②膻中、关元、中脘、天枢、乳根;③丰隆、脾俞、风门;④大椎、太渊、太溪、列缺、尺则。结论针刺治疗COPD稳定期的选穴规律以局部选穴和循经选穴为主,以特定穴选用和腧穴之间配伍为辅,体现了中医“扶正祛邪、标本兼顾”的治疗原则。Objective To explore characteristics and rules of clinical acupoint selection during treatment of chronic obstructive pulmonary disease(COPD)at stable stage.Methods The paper retrieved database,screened literature according to inclusion and exclusion criteria,extracted information of acupuncture prescription and acupoint meridian tropism to establish the database,and analyzed frequency,meridian tropism,distribution,related rules and cluster of acupuncture point selection with data mining methods including Apriori association rule and systematic cluster analysis and etc.Results The paper included 63 articles,extracted 55 acupuncture prescriptions,involving 53 acupoints,with cumulative frequency of 406 times.Top three acupoints with frequency of use were Feiyu,Danzhong and Zusanli.Main meridians included taiyang bladder channel of foot,foot yangming stomach meridian of ren meridian,lung channel of hand-taiyin.Most of acupoint selection sites involved chest,abdomen,waist and back,most selected special points were Beiyu,Wushu,Mu and Bahui.There are 10 strong association rules based on related rule analysis,including Shenyu,Feiyu,Dingchuan+Zusanli,Feiyu,Shenyu+Zusanli,Feiyu,and etc.Four cluster groups were obtained by cluster analysis,namely;①Feiyu,Dingchuan and Zusanli;②Danzhong,Guanyuan,Zhongwan,Tianshu and Rugen;③Honglong,Piyu and Fengmen;④Dazhui,Taiyuan,Taixi,Lieque and Chize.Conclusion Acupuncture treatment of COPD at stable stage is mainly based on local acupoint selection and acupoint selection along meridians,supplemented by selection of specific acupoints and compatibility between acupoints,which embodies treatment principle of"strengthening body resistance and eliminating pathogens,considering both symptoms and root causes"of traditional Chinese medicine.
