摘  要:目的探讨改良脑血管造影导管在经桡动脉路径(TRA)全脑血管造影中的有效性及安全性。方法回顾性分析2022年6月至2024年6月在南阳豫西健和医院进行全脑血管造影检查的102例患者的临床资料,按照全脑血管造影导管选择不同将其分为改良导管组54例和西蒙Ⅲ导管组48例。比较两组患者动脉插管的成功率、曝光和造影时间以及并发症的发生情况。结果改良导管组患者的左侧椎动脉插管成功率为87.94%,明显高于西蒙Ⅲ导管组的56.25%,差异有统计学意义(P<0.05),但改良导管组患者的左侧颈总动脉、右侧椎动脉、右侧颈总动脉插管成功率分别为94.44%、98.15%、98.15%,分别与西蒙Ⅲ导管组的85.42%、91.67%、89.58%比较差异均无统计学意义(P>0.05);改良导管组患者的造影及曝光时间分别为(21.04±3.21)min、(14.52±2.50)min,明显短于西蒙Ⅲ导管组的(25.17±3.70)min、(17.86±2.03)min,差异均有统计学意义(P<0.05);改良导管组患者的并发症总发生率为1.85%,明显低于西蒙Ⅲ导管组的12.50%,差异有统计学意义(P<0.05)。结论改良脑血管造影导管在TRA全脑血管造影中的应用能够提高患者左侧椎动脉插管的成功率,缩短造影和曝光时间,安全性高。Objective To explore the efficacy and safety of improved catheter in whole cerebral angiography via transradial artery(TRA)access.Methods The clinical data of 102 patients who underwent whole cerebral angiography from June 2022 to June 2024 in Nanyang Yuxi Jianhe Hospital were retrospectively analyzed.According to the selection of catheters for whole cerebral angiography,the patients were divided into improved catheter group(n=48)and SimonⅢcatheter group(n=48).The success rate of arterial intubation,the exposure time,and the angiography time,and the occurrence of complications were compared between the two groups.Results The success rate of intubation in left vertebral artery in the improved catheter group was 87.94%,which was significantly higher than 56.25%of SimonⅢcatheter group(P<0.05).The success rates of intubation in left common carotid artery,right vertebral artery,and right common carotid artery in the modified catheter group were 94.44%,98.15%,and 98.15%,which showed no significant difference with 85.42%,91.67%,and 89.58%in the SimonⅢcatheter group(P>0.05).The time of angiography and exposure in the improved catheter group were(21.04±3.21)min and(14.52±2.50)min,which were significantly shorter than(25.17±3.70)min and(17.86±2.03)min in SimonⅢcatheter group(P<0.05).The total incidence of complications in the improved catheter group was 1.85%,which was significantly lower than 12.50%in SimonⅢcatheter group(P<0.05).Conclusion The application of improved catheter in whole cerebral angiography via TRA can improve the success rate of left vertebral artery catheterization in patients,shorten the imaging and exposure time,and have high safety.
