血栓弹力图参数联合IL-12、25-(OH)D检测对妊娠期高血压患者的诊断价值

摘　　要：目的探讨血栓弹力图参数联合白细胞介素-12(IL-12)、25-羟维生素D[(25-(OH)D)]检测对妊娠期高血压(GH)患者的诊断价值。方法前瞻性选取2022年1月至2024年2月郑州市第七人民医院收治的81例GH患者作为研究组,70例正常妊娠孕妇作为对照组。比较两组孕妇的血栓弹力图参数[凝血反应时间(R)、凝固时间(K)、凝固角(α角)、血栓最大振幅(MA)]、血清IL-12、25-(OH)D水平,并采用受试者工作特征(ROC)曲线分析血栓弹力图参数联合IL-12、25-(OH)D对GH患者的诊断价值。结果研究组孕妇的R、K分别为(5.35±1.26)min、(1.14±0.21)min,明显低于对照组的(6.81±1.40)min、(2.32±0.33)min,α角、MA分别为(76.30±11.25)°、(76.20±10.28)mm,明显高于对照组的(60.47±10.31)°、(60.78±9.44)mm,差异均有统计学意义(P<0.05);研究组孕妇的血清IL-12水平为(62.87±5.20)pg/mL,明显高于对照组的(50.01±3.62)pg/mL,25-(OH)D水平为(14.54±3.46)mg/mL,明显低于对照组的(22.05±4.86)mg/mL,差异均有统计学意义(P<0.05);ROC曲线分析结果显示,血栓弹力图参数(R、K、α角、MA)、IL-12、25-(OH)D联合检测AUC为0.940,高于各指标单一预测。结论血栓弹力图参数联合IL-12、25-(OH)D检测能提高早期诊断GH患者的准确性,有助于制定治疗方案。Objective To explore the diagnostic value of thromboelastography parameters combined with interleukin-12(IL-12)and 25-hydroxyvitamin D[25-(OH)D]in patients with gestational hypertension(GH).Methods Eighty-one GH patients admitted to Zhengzhou Seventh People's Hospital from January 2022 to February 2024 were prospectively selected as the study group,and 70 normal pregnant women were selected as the control group.The thromboelastography parameters(coagulation reaction time[R],coagulation time[K],coagulation angle[αangle],maximum amplitude of thrombi[MA]),serum IL-12,25-(OH)D levels were compared between the two groups.Receiver operating characteristic(ROC)curve was used to analyze the diagnostic value of elastography parameters combined with IL-12 and 25-(OH)D in GH patients.Results The R and K of the study group were(5.35±1.26)min and(1.14±0.21)min,which were significantly lower than(6.81±1.40)min and(2.32±0.33)min of the control group;αangle and MA were(76.30±11.25)°and(76.20±10.28)mm,respectively,significantly higher than(60.47±10.31)°and(60.78±9.44)mm in the control group;the differences were statistically significant(P<0.05).The serum IL-12 level of pregnant women in the study group was(62.87±5.20)pg/mL,which was significantly higher than(50.01±3.62)pg/mL in the control group;the 25-(OH)D level was(14.54±3.46)mg/mL,which was significantly lower than(22.05±4.86)mg/mL of the control group;the differences were statistically significant(P<0.05).ROC curve analysis showed that the AUC of the combined detection of thrombologram parameters(R,K,αangle,MA),IL-12,and 25-(OH)D was 0.940,which was higher than that of each index alone.Conclusion The value of thrombologram parameters combined with IL-12 and 25-(OH)D detection is significant in the diagnosis of GH patients,which can improve the accuracy of early diagnosis and predict the disease progression,thus contributing to the formulation of treatment plans.
