摘　　要：目的探讨罗哌卡因联合右美托咪定硬膜外麻醉对子宫下段剖宫产术患者的镇痛效果及新生儿结局的影响。方法选取2021年3月—2023年3月在本院接受剖宫产手术的98例产妇作为观察对象,按照接收时间将其分为两组,各49例。对照组予以罗哌卡因硬膜外麻醉,实验组予以罗哌卡因联合右美托咪定硬膜外麻醉,比较两组临床效果。结果实验组首次泌乳时间、麻醉起效和达峰时间短于对照组(P<0.05),麻醉平面持续时间长于对照组(P<0.05),麻醉后20min及胎儿娩出时实验组镇静效果优于对照组(P<0.05);两组脐动脉血气指标及Apgar评分均无明显差异(P>0.05)。结论将罗哌卡因联合右美托咪定硬膜外麻醉应用于剖宫产手术,能够提高麻醉质量,提高镇静效果,促进术后康复,对新生儿影响较小,安全性可靠,具有重要应用意义。Objective To explore analgesic effect of ropivacaine combined with dexmedetomidine epidural anesthesia for patients with lower segment cesarean section and its effect on neonatal outcomes.Methods The paper chose 98 parturients with cesarean section surgery in our hospital from March 2021 to March 2023 as observation subjects,and divided them into two groups according to reception time,with 49 cases in each group.Control group was treated with epidural anesthesia with ropivacaine,while experimental group with epidural anesthesia with ropivacaine combined with dexmedetomidine.Clinical effect between two groups was compared.Results First lactation time,anesthesia onset and peak time of experimental group was shorter than control group(P<0.05),and duration of anesthesia level was longer than control group(P<0.05).Sedative effect of experimental group was better than control group at 20 minutes after anesthesia and fetal delivery(P<0.05).There was no significant difference in umbilical artery blood and Apgar scores between two groups(P>0.05).Conclusion Ropivacaine combined with dexmedetomidine epidural anesthesia can improve quality of anesthesia,sedative effect,promote postoperative recovery of patients with cesarean section surgery,with minimal effect on newborns,which is safe and reliable and has important application significance.
