摘　　要：分析产前超声筛查胎儿先天性心脏病(congenital heart disease,CHD)的价值。选取黔东南州人民医院2022年1月-2023年7月收治的1000例高危孕妇作为观察对象,所有孕妇接受超声扫描,包括常规超声、系统超声,以产后或者引产结果为标准,评价不同超声技术的诊断准确性及胎儿患CHD的具体情况,并分析诱发CHD的危险因素。研究结果显示,1000例孕妇中,胎儿患有CHD的有13例,常规超声检查8例(准确率为61.53%)、系统超声检查12例(准确率为92.31%),系统超声检查准确率高于常规超声,差异有统计学意义(P<0.05)。自多因素Logistic回归分析,流产超过2次、妊娠期合并症、孕期使用抗生素、24 h蛋白尿超过3 g、孕期被动吸烟等是引起胎儿CHD的独立危险因素。研究发现,采取产前系统超声诊断胎儿CHD的准确率高,可及时发现胎儿心脏异常,为早期终止妊娠提供支持。同时,明确诱发胎儿CHD的危险因素后,需实施预防性措施,以降低疾病发生概率。Analyze the value of prenatal ultrasound screening for congenital heart disease(CHD)in fetuses.Selecting 1000 high-risk pregnant women admitted to Qiandongnan Prefecture People’s Hospital from January 2022 to July 2023 as observation subjects,all pregnant women received ultrasound scans,including routine ultrasound and systematic ultrasound.Based on postpartum or induced labor results,the diagnostic accuracy of different ultrasound techniques and the specific situation of fetal CHD were evaluated,and the risk factors for inducing CHD were analyzed.The results showed that among 1000 pregnant women,13 cases had CHD in their fetuses.Among them,8 cases underwent routine ultrasound examination(accuracy rate of 61.53%)and 12 cases underwent systematic ultrasound examination(accuracy rate of 92.31%).The accuracy of systematic ultrasound examination was higher than that of routine ultrasound,and the difference was statistically significant(P<0.05).Multiple logistic regression analysis showed that more than 2 miscarriages,pregnancy complications,use of antibiotics during pregnancy,24-hour proteinuria exceeding 3g,and passive smoking during pregnancy are independent risk factors for fetal CHD.Research has found that the accuracy of prenatal ultrasound diagnosis for fetal CHD is high,which can timely detect fetal cardiac abnormalities and provide support for early termination of pregnancy.At the same time,after identifying the risk factors for inducing fetal CHD,preventive measures should be implemented to reduce the probability of disease occurrence.
