人工关节置换术及内固定术治疗老年股骨转子间骨折的临床效果研究

摘　　要：讨论人工关节置换术、内固定术用于老年股骨转子间骨折治疗的价值。选取2022年4月-2023年4月剑河县人民医院收治的接受手术治疗的92例老年股骨转子间骨折患者为研究对象,以随机数字表法将其分为参照组和试验组,每组样本容量均为46例。参照组实施股骨近端防旋髓内钉内固定术治疗,试验组实施人工关节置换术治疗,对两组患者髋关节功能、凝血功能、手术相关指标、并发症发生率、SF-36进行评定。研究结果表明,治疗后,试验组疼痛评分低于参照组,髋关节功能、活动度指标优于参照组(P<0.05);试验组D二聚体水平低于参照组(P<0.05);试验组术后并发症发生率低于参照组(P<0.05);治疗后,试验组SF-36评分高于参照组(P<0.05)。研究发现,人工关节置换术在老年股骨转子间骨折患者的治疗中均具有显著优势,可降低患者术后并发症发生率,加快患者髋关节功能改善速度,提高患者生活质量。Discuss the value of artificial joint replacement and internalfixation in the treatment of intertrochanteric fractures in elderly patients.92 elderly patients with intertrochanteric fractures who underwent surgical treatment at Jianhe People’s Hospital from April 2022 to April 2023 were selected as the study subjects.They were randomly divided into a reference group and an experimental group using a random number table method,with a sample size of 46 cases in each group.The reference group underwent proximal femoral anti rotation intramedullary nailfixation surgery,while the experimental group underwent artificial joint replacement surgery.The hip joint function,coagulation function,surgical related indicators,incidence of complications,and SF-36 were evaluated for both groups of patients.The results showed that after treatment,the pain score of the experimental group was lower than that of the reference group,and the hip joint function and range of motion indicators were better than those of the reference group(P<0.05);The D-dimer level in the experimental group was lower than that in the reference group(P<0.05);The incidence of postoperative complications in the experimental group was lower than that in the reference group(P<0.05);After treatment,the SF-36 score of the experimental group was higher than that of the reference group(P<0.05).Research has found that artificial joint replacement surgery has significant advantages in the treatment of elderly patients with intertrochanteric fractures of the femur.It can reduce the incidence of postoperative complications,accelerate the improvement of hip joint function,and improve the quality of life of patients.
