摘　　要：目的 探究急性病毒性肝炎(AVH)患者血清组蛋白脱乙酰酶3(HDAC3)和组蛋白脱乙酰酶11(HDAC11)表达水平与T细胞淋巴亚群的关系。方法 选取2022年1月—2023年6月在秦皇岛市第三医院就诊的120例AVH患者作为研究对象(AVH组),并选择同期的120名体检健康者作为对照组。收集并比较两组的一般临床资料。检测两组血清中HDAC3、HDAC11、CD3^(+)、CD4^(+)和CD4^(+)/CD8^(+)水平。利用Pearson等级相关性分析AVH患者血清HDAC3、HDAC11水平与T细胞淋巴亚群的相关性。利用受试者工作特征(ROC)曲线分析血清HDAC3、HDAC11对诊断AVH的临床价值。结果 AVH组和对照组的年龄、性别和BMI无显著性差异(P>0.05)。与对照组相比,AVH组的丙氨酸转氨酶、天冬氨酸转氨酶、总胆红素、三酰甘油、胆固醇、低密度胆固醇显著升高(P<0.05),而AVH组的高密度胆固醇显著降低(P<0.05)。AVH组患者的血清HDAC3和HDAC11表达水平均显著高于对照组(P<0.05),CD3^(+)、CD4^(+)和CD4^(+)/CD8^(+)均显著降低于对照组(P<0.05)。Pearson相关分析显示,AVH患者血清HDAC3水平与CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)均呈负相关(r=-0.458、-0.533、-0.462,均P<0.001),HDAC3表达水平与CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)均呈负相关(r=-0.473、-0.551、-0.422,均P<0.001)。ROC曲线显示,HDAC3、HDAC11及二者联合预测诊断AVH的曲线下面积(AUC)分别为0.921、0.941、0.988,血清HDAC3、HDAC11联合诊断AVH的临床应用价值高于单项指标诊断(Z_(二者联合-HDAC3)=3.672、P=0.000;Z_(二者联合-HDAC11)=3.078、P=0.002)。结论 AVH患者血清HDAC3和HDAC11表达水平升高,且与T淋巴细胞亚群存在相关性,HDAC3与HDAC11联合预测AVH的临床价值较高。ObjectiveTo explore the relationship between the expression levels of serum histone deacetylase 3(HDAC3) and histone deacetylase 11 (HDAC11) and T lymphocyte subsets in patients with acute viral hepatitis (AVH).Methods120 AVH patients who visited our hospital from January 2022 to June 2023 were collected as the study subjects (AVH group),and 120 healthy individuals who underwent physical examination were as the control group.The general clinical data of the two groups were collected and compared.The levels of HDAC3,HDAC11,CD3~+,CD4~+,and CD4~+/CD8~+in the serum of two groups were detected.Pearson rank correlation was applied to analyze the correlation between serum HDAC3,HDAC11 levels and T cell lymphoid subsets in AVH patients.Receiver operating characteristic (ROC) curve was applied to analyze the clinical value of serum HDAC3 and HDAC11 in the diagnosis of AVH.ResultsThere was no obvious difference in age,gender,and BMI between the AVH group and the control group (P>0.05).Compared with the control group,the alanine transaminase,glutamic transaminase,total bilirubin,triglycerides,cholesterol,low density cholesterol in the AVH group increased obviously (P<0.05),while high-density cholesterol in the AVH group decreased obviously (P<0.05).The expression levels of serum HDAC3 and HDAC11 in the AVH group were obviously higher than those in the control group (P<0.05),while the levels of CD3~+,CD4~+,and CD4~+/CD8~+ were obviously lower than those in the control group (P<0.05).Pearson correlation analysis showed that serum HDAC3 level in AVH patients was negatively correlated with CD3~+,CD4~+,and CD4~+/CD8~+(r=-0.458,-0.533,-0.462,P<0.001),while HDAC3 expression level was negatively correlated with CD3~+,CD4~+,and CD4~+/CD8~+(r=-0.473,-0.551,-0.422,P<0.001).Receiver operating characteristic showed that the area under the curve (AUC) of HDAC3,HDAC11 and their combination in predicting AVH was 0.921,0.941 and 0.988,respectively,the clinical application value of serum HDAC3 combined HDAC11 in diagnosing AVH was higher tha
