基于新生儿急性疼痛评估量表的新生儿疼痛影响因素分析

摘　　要：目的基于新生儿急性疼痛行为评分量表(NIAPAS)分析新生儿疼痛影响因素。方法选取2023年1月至12月于深圳市儿童医院住院的80例新生儿作为研究对象,使用NIAPAS进行评估,分析新生儿疼痛影响因素。结果单因素分析显示,NIAPAS>6分新生儿日龄≤7d、穿刺、辅助呼吸、有创检查、静脉标本采集部位占比均高于NIAPAS≤6分新生儿,经阴道分娩率低于NIAPAS≤6分新生儿(P<0.05);NIAPAS>6分与NIAPAS≤6分新生儿性别、胎龄、出生体重比较,差异无统计学意义(P>0.05)。logistic回归分析显示,分娩方式、日龄、穿刺、辅助呼吸、有创检查、标本采集部位均属于新生儿疼痛的影响因素(P<0.05)。结论使用NIAPAS对新生儿疼痛影响因素进行分析,发现分娩方式、日龄、穿刺、辅助呼吸、有创检查、标本采集部位均与新生儿疼痛密切相关,临床可针对上述因素制订合理的干预措施。Objective To analyze the influencing factors of neonatal pain based on neonatal infant acute pain assessment scale(NIAPAS).Methods A total of 80 neonates in Shenzhen Children’s Hospital from January to December 2023 were selected as the study objects.The NIAPAS was used to evaluate the neonatal pain,and the influencing factors of neonatal pain were analyzed.Results Univariate analysis showed that the neonates with NIAPAS>6 points had higher rates of age≤7 days,puncture,assisted respiration,invasive examination,and venous specimen collection than those with NIAPAS≤6 points,and had lower rate of vaginal delivery than that with NIAPAS≤6 points(P<0.05).There were no significant differences in gender,gestational age,and birth weight between neonates with NIAPAS>6 points and NIAPAS≤6 points(P>0.05).The logistic regression analysis showed that delivery mode,age in days,puncture,assisted breathing,invasive examination,and specimen collection site were all factors affecting neonatal pain(P<0.05).Conclusion NIAPAS is used to analyze the influencing factors of neonatal pain,and it is found that delivery mode,age in days,puncture,assisted breathing,invasive examination,and specimen collection site are closely related to neonatal pain,and reasonable intervention measures could be formulated according to the above factors.
