苦参凝胶对阴道内假丝酵母菌的敏感性研究

摘  要:目的:观察外阴阴道假丝酵母菌病(VVC)患者的假丝酵母菌临床分离株对苦参凝胶的敏感性,为临床治疗不同类型VVC提供科学依据。方法:选取VVC患者阴道分泌物来源白假丝酵母菌咪康唑敏感菌株、白假丝酵母菌咪康唑耐药、非白假丝酵母菌咪康唑敏感株、非白假丝酵母菌咪康唑耐药各30株。参照美国临床实验室标准化协会(CLSI)方案,测定其对苦参凝胶的最低抑菌浓度(MIC)值情况。结果:苦参凝胶对白假丝酵母菌和非白假丝酵母菌,包括咪康唑耐药菌株在内的所有VVC临床菌株均具有抑制作用。白假丝酵母菌咪康唑敏感MIC为1.56~6.25mg/mL,耐药MIC为1.56~6.25 mg/mL;非白假丝酵母菌咪康唑敏感MIC为1.56~12.5mg/mL,耐药MIC为3.125~12.5 mg/mL。在白假丝酵母菌中,咪康唑敏感组的MIC(3.18±1.56)与耐药组(3.85±1.52)差异无统计学意义;在非白假丝酵母菌中,咪康唑敏感组的MIC(5.99±2.25)与耐药组(7.19±2.48)差异有统计学意义(P<0.05);在咪康唑敏感组中,白假丝酵母菌MIC(3.18±1.56)与非白假丝酵母菌(5.99±2.25)差异有统计学意义(P<0.001)。在咪康唑耐药组,白假丝酵母菌(3.85±1.52)与非白假丝酵母菌(7.19±2.48)差异有统计学意义(P<0.001)。结论:苦参凝胶能有效抑制白假丝酵母菌和非白假丝酵母菌,尤其是对于白假丝酵母菌的抑制效果更好,对于咪康唑耐药的VVC菌株也具有良好的抗菌活性,提示苦参凝胶治疗VVC患者具有较好的临床价值。Objective:To observe the sensitivity of clinical isolates of Candida from vulvovaginal candidiasis(VVC)patients to Sophora gel,providing scientific evidence for the treatment of different types of VVC.Methods:Thirty strains each of Candida albicans susceptible to miconazole,miconazole-resistant Candida albicans,miconazole-susceptible non-albicans Candida,and miconazole-resistant non-albicans Candida were selected from vaginal secretions of VVC patients.Following the Clinical and Laboratory Standards Institute(CLSI)protocol,their minimum inhibitory concentrations(MICs)to Sophora gel were determined.Results:Sophora gel inhibited all clinical strains of VVC,including Candida albicans and non-Candida albicans species,as well as miconazole-resistant strains.The MIC for miconazole-sensitive Candida albicans was 1.56~6.25 mg/mL,and for miconazole-resistant Candida albicans,it was also 1.56~6.25 mg/mL.The MIC for miconazole-sensitive non-Candida albicans species ranged from 1.56 to 12.5mg/mL,while for miconazole-resistant non-Candida albicans species,it ranged from 3.125 to 12.5 mg/mL.For Candida albicans,no statistical difference was observed between the miconazole-sensitive group(3.18±1.56)and the miconazole-resistant group(3.85±1.52).A statistical difference(P<0.05)was observed between the miconazole-sensitive group(5.99±2.25)and the miconazole-resistant group(7.19±2.48)for non-Candida albicans species.Within the miconazole-sensitive group,a statistical difference(P<0.001)was observed between Candida albicans(3.18±1.56)and non-Candida albicans species(5.99±2.25).Within the miconazole-resistant group,a statistical difference(P<0.001)was also observed between Candida albicans(3.85±1.52)and non-Candida albicans species(7.19±2.48).Conclusion:Sophora gel effectively inhibits both Candida albicans and non-albicans Candida,with superior inhibition against Candida albicans.It also demonstrates good antibacterial activity against miconazole-resistant VVC strains,suggesting its promising clinical value in VVC treatment.
