盐酸赛洛唑啉联合布地奈德治疗变应性鼻炎的临床研究

摘　　要：目的:分析盐酸赛洛唑啉联合布地奈德对变应性鼻炎(Allergic rhinitis,AR)患者临床症状及鼻腔生理功能指标的影响。方法:选取2022年1月至2023年5月我院收治的138例AR患者,随机分为对照组和观察组,每组各69例。对照组采用布地奈德治疗,单鼻孔每次64μg,早晚各喷1次。观察组采用盐酸赛洛唑啉联合布地奈德治疗,盐酸赛洛唑啉每次2-3喷,每日2次。比较两组的疗效、临床症状、不良反应,治疗7 d后采用糖精清除试验测定鼻黏膜纤毛传输时间(Mucociliary transport time,MTT),采用鼻阻力仪测定鼻阻力(Nasal airway resistance,NR),采用酶联免疫法测定嗜酸性粒细胞阳离子蛋白(Eosinophil cationic protein,ECP)、总免疫球蛋白E(Total immunoglobulin E,TIgE)、血清黏附分子可溶性细胞间黏附分子-1(Soluble intercellular adhesion molecule-1,sICAM-1)、P选择素(P-selectin,PS)、白介素-4(Interleukin-4,IL-4)、干扰素-γ(Interferon-γ,IFN-γ),计算IFN-γ/IL-4值。结果:观察组临床总有效率显著高于对照组(P<0.05)。治疗后,观察组的鼻塞、鼻痒、喷嚏、流涕及临床症状总分均比对照组显著降低(P<0.05)。观察组治疗后IFN-γ、IFN-γ/IL-4高于对照组,MTT、NR、sICAM-1、ECP、PS、TIgE、IL-4低于对照组(P<0.05);两组不良反应发生率无显著差异(P>0.05)。结论:盐酸赛洛唑啉联合布地奈德治疗AR,能提高疗效,减轻患者的临床症状,改善鼻腔生理功能,调节机体免疫平衡,减轻炎症反应,且安全性高。Objective:To analyze the effect of the combination of Xylometazoline Hydrochloride and Budesonide on the clinical symptoms and nasal physiological function indicators of patients with allergic rhinitis(AR).Methods:A total of 138 patients with AR admitted to our hospital from January 2022 to May 2023 were randomly divided into a control group and an observation group,with 69 cases in each group.The control group was treated with budesonide,with 64μg per nostril,sprayed once in the morning and evening.The observation group was treated with salbutamol hydrochloride combined with budesonide,with 2-3 sprays of salbutamol hydrochloride per time,twice a day.The efficacy,clinical symptoms,and adverse reactions of the two groups were compared.After 7 days of treatment,the saccharin clearance test was used to measure the mucociliary transport time(MTT)of nasal mucosa,the nasal airway resistance(NR)was measured using a nasal resistance meter,and the eosinophil cationic protein(ECP),total immunoglobulin E(TIgE),serum adhesion molecules[soluble intercellular adhesion molecule-1(sICAM-1),P-selectin(PS),interleukin-4(IL-4),interferon-γ(IFN-γ)]were measured using enzyme-linked immunosorbent assay.The IFN-γ/IL-4 value was calculated.Results:The total clinical effective rate in the observation group was significantly higher than that in the control group(P<0.05).After treatment,the total scores of nasal congestion,itching,sneezing,runny nose,and clinical symptoms in the observation group were significantly lower than those in the control group(P<0.05).After treatment,the MCV,IFN-γ,and IFN-γ/IL-4 in the observation group were higher than those in the control group,while the MTT,NR,sICAM-1,ECP,PS,TIgE,and IL-4 were lower than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The combination of cylozoline hydrochloride and budesonide in the treatment of AR can improve the efficacy,reduce the clinical symptoms,improve the nasal p
