摘 要:目的:探讨母体血清血管内皮生长因子(Vascular endothelial growth factor,VEGF)、促红细胞生成素(Erythropoietin,EPO)、趋化因子12(Chemokine 12,CXCL12)水平联合检测对胎儿宫内窘迫的诊断价值。方法:选取2022年1月-2024年1月期间本院收治的105例胎儿宫内窘迫孕妇为研究对象。同时,选取同期于本院产检的健康孕妇105例为对照组。依据宫内窘迫程度将产妇分为轻度组(64例)和中重度组(41例)。比较三组血清VEGF、EPO、CXCL12水平、平均胎心率、脐动脉血流[收缩期和舒张期比值(Systolic to diastolic ratio,S/D)、阻力指数(Resistance index,RI)、搏动指数(Pulsatility index,PI)],分析其相关性及联合检测对胎儿宫内窘迫的诊断价值。结果:中重度组血清VEGF、EPO、CXCL12水平均显著高于轻度组、对照组(P<0.05);轻度组血清VEGF、EPO、CXCL12水平均显著高于对照组(P<0.05)。中重度组的平均胎心率、脐动脉血流均显著高于轻度组、对照组;轻度组的平均胎心率、脐动脉血流均显著高于对照组(P<0.05)。血清VEGF、EPO、CXCL12水平与胎儿宫内窘迫程度、平均胎心率、脐动脉血流均呈正相关,差异有统计学意义(P<0.05)。血清VEGF、EPO、CXCL12水平联合诊断胎儿宫内窘迫的AUC为0.920,约登指数为0.724,敏感度为91.43%,特异度为80.95%,差异有统计学意义(P<0.05)。结论:母体血清VEGF、EPO、CXCL12水平与胎儿宫内窘迫及其病情程度相关,三者联合检测可提高对胎儿宫内窘迫的诊断价值。Objective:To investigate the diagnostic value of maternal serum vascular endothelial growth factor(VEGF),erythropoietin(EPO)and chemokine 12(CXCL12)levels in the diagnosis of fetal distress.Methods:105 pregnant women with fetal distress admitted to our hospital from January 2022 to January 2024 were selected as the study objects.At the same time,105 healthy pregnant women were selected as the control group.According to the degree of intrauterine distress,the women were divided into mild group(64 cases)and moderate and severe group(41 cases).Serum levels of VEGF,EPO,CXCL12,average fetal heart rate,umbilical artery flow[systolic to diastolic ratio(S/D),resistance index(RI),pulse index(PI)]of the three groups were compared to analyze their correlation and the diagnostic value of combined detection in fetal distress.Results:Serum levels of VEGF,EPO and CXCL12 in moderate and severe group were significantly higher than those in mild group and control group(P<0.05).Serum levels of VEGF,EPO and CXCL12 in mild group were significantly higher than those in control group(P<0.05).The average fetal heart rate and umbilical artery flow in moderate and severe group were significantly higher than those in mild group and control group.The mean fetal heart rate and umbilical artery flow in mild group were significantly higher than those in control group(P<0.05).Serum levels of VEGF,EPO and CXCL12 were positively correlated with intrauterine distress degree,mean fetal heart rate and umbilical artery flow,with statistical significance(P<0.05).The AUC of serum VEGF,EPO and CXCL12 in the combined diagnosis of fetal distress was 0.920,the Yoeden index was 0.724,the sensitivity was 91.43%,and the specificity was 80.95%,the differences were statistically significant(P<0.05).Conclusion:The levels of maternal serum VEGF,EPO and CXCL12 are related to the severity of fetal distress,and the combined detection of the three can improve the diagnostic value of fetal distress.
