血清TGF-β1、NGAL水平对糖尿病患者早期肾损伤的评估分析

摘　　要：目的:探析转化生长因子(Transforming Growth Factor-beta,TGF-β1)、中性粒细胞明胶酶相关脂质运载蛋白(Neutrophil Gelatinase associated Lipocalin,NGAL)水平对糖尿病患者早期肾损伤的评估意义。方法:选取2022年1月至2023年1月期间本院检验科的68例糖尿病患者作为研究对象。对所有患者随访6 m评估早期肾损伤情况,并依据肾损伤发生情况将患者分为发生组与未发生组。检测患者入院时血清TGF-β1、NGAL水平,分析血清TGF-β1、NGA水平对糖尿病患者早期肾损伤的评估价值。结果:68例糖尿病患者中发生肾损伤有14例,发生率为20.59%。发生组血清TGF-β1水平高于未发生组,NGAL水平低于未发生组,差异有统计学意义(P<0.05);经点二列相关性分析显示,TGF-β1水平与糖尿病患者早期肾损伤的发生呈正相关(r>0,P<0.05),而NGAL水平与糖尿病患者早期肾损伤的发生呈负相关(r<0,P<0.05);绘制受试者工作特征曲线(Receiver Operating Characteristic,ROC),血清TGF-β1、NGAL单独及联合评估糖尿病患者早期肾损伤的曲线下面积(Area Under Curve,AUC)均>0.70,其中以联合评估价值最高。结论:血清TGF-β1、NGAL异常表达参与糖尿病患者早期肾损伤发生,对糖尿病患者早期肾损伤评估具有较高价值。Objective:To investigate the significance of transforming growth factor(Transforming Growth Factor-beta,TGF-β1)and neutrophil gelatinase-associated lipid carrier protein(Neutrophil Gelatinase associated Lipocalin,NGAL)levels in the assessment of early renal injury in diabetic patients.Methods:68 diabetic patients in the clinical laboratory of our hospital from January 2022 to January 2023 were selected as research objects.All patients were followed up for 6 months to evaluate the early kidney injury,and the patients were divided into two groups according to the occurrence of kidney injury.Serum TGF-β1 and NGAL levels were detected at admission,and the evaluation value of serum TGF-β1 and NGA levels on early kidney injury in diabetic patients was analyzed.Results:Among 68 diabetic patients,14 cases(20.59%)had renal injury.The serum TGF-β1 level in the occurrence group was higher than that in the non-occurrence group,and the NGAL level was lower than that in the non-occurrence group,the difference was statistically significant(P<0.05).TGF-β1 level was positively correlated with the occurrence of early kidney injury in diabetic patients(r>0,P<0.05),while NGAL level was negatively correlated with the occurrence of early kidney injury in diabetic patients(r<0,P<0.05).According to receiver operating characteristic curve(ROC),the area under the curve(AUC)of serum TGF-β1 and NGAL alone and combined in assessing early kidney injury in diabetic patients were all greater than 0.70,and the value of combined assessment was the highest.Conclusion:The abnormal expression of TGF-β1 and NGAL in serum is involved in the early renal injury in diabetic patients,and has a high value in the assessment of early renal injury in diabetic patients.
