经阴道超声、经腹部超声诊断剖宫产术后瘢痕妊娠的价值分析

摘　　要：目的:探讨剖宫产术后瘢痕妊娠经阴道与经腹部超声诊断的价值。方法:回顾性收集2021年1月~2023年2月期间于本院行剖宫产后瘢痕妊娠接受手术治疗的87例患者的临床资料。所有患者术前先经腹部超声检查,然后行经阴道超声检查。以手术结果作为诊断瘢痕妊娠的金标准,比较术前经腹部超声检查、经腹部与经阴道超声联合诊断瘢痕妊娠的价值。结果:87例瘢痕妊娠患者,经手术确诊Ⅰ型瘢痕妊娠62例,Ⅱ型瘢痕妊娠25例。术前经腹部与经阴道超声联合诊断瘢痕妊娠的准确率及对瘢痕妊娠类型的鉴别准确率均高于经腹部超声、经阴道超声单一检查(P<0.05),且联合鉴别诊断的结果与“金标准”一致性强(P<0.05)。结论:应用经阴道与经腹部超声联合诊断剖宫产术后瘢痕妊娠的价值较高,且联合检查对瘢痕妊娠类型的诊断亦更为准确。Objective:To evaluate the value of transvaginal and transabdominal ultrasonography in the diagnosis of cicatricial pregnancy after cesarean section.Methods:The clinical data of 87 patients who underwent surgical treatment for cicatricial pregnancy after cesarean section from January 2021 to February 2023 were retrospectively collected.All patients underwent abdominal ultrasonography before surgery and then transvaginal ultrasonography.Surgical and pathological results are the gold standard for diagnosis of scar pregnancy.To compare the value of preoperative transabdominal ultrasonography and combined transabdominal and transvaginal ultrasonography in diagnosis of scar pregnancy.Results:Among 87 patients with scar pregnancy,62 cases of type I scar pregnancy and 25 cases of type II scar pregnancy were confirmed by operation.The accuracy of combining transabdominal and transvaginal ultrasound and the differentiation accuracy of scar pregnancy types were higher than that of transabdominal ultrasound and transvaginal ultrasound(P<0.05),and the results of combined differential diagnosis were consistent with the"gold standard"(P<0.05).Conclusion:The value of combined transvaginal and transabdominal ultrasonography in the diagnosis of scar pregnancy after cesarean section is higher,and the combined examination is more accurate in the diagnosis of scar pregnancy types.
