摘　　要：目的:研究后外侧入路和后内侧入路对Rüedi-AllgwerⅡ、Ⅲ型Pilon骨折患者围术期指标、骨折愈合的影响。方法:选取2019年2月~2024年8月本院收治的60例Rüedi-AllgwerⅡ、Ⅲ型Pilon骨折患者为研究对象,按抛硬币法随机分为采用后内侧入路固定术的内侧组(n=30)及采用后外侧入路的外侧组(n=30)。比较两组患者治疗期间各围术期指标,记录骨折愈合时长,对比踝关节功能并记录术后并发症发生情况。结果:内侧组患者切口长度、术中出血量、手术时间以及骨折愈合时间均低于外侧组(P<0.05);内侧组美国足踝外科协会踝-后足(AOFAS)评分高于外侧组(P<0.05);术后两组患者并发症发生率无明显差异(P>0.05)。结论:后内侧入路相较于后外侧入路治疗效果更好,可促进骨折快速愈合,提高愈合效果,有效提高踝关节功能,安全性好。Objective:To investigate the effects of posterior lateral approach and posteromedial approach on perioperative indicators and fracture healing in patients with Ruedi-Allgower type Ⅱ/Ⅲ Pilon fractures.Methods:Seventy patients with Ruedi-Allgower type Ⅱ/Ⅲ Pilon fractures who were admitted to the hospital from February 2019 to August 2024 were selected as study subjects.They were randomly divided into the medial group(n=30,fixation via posteromedial approach)and the lateral group(n=30,fixation via posterior lateral approach).Perioperative indicators,fracture healing time,ankle function,and the incidence of postoperative complications were compared between the two groups.Results:The length of incision,intraoperative blood loss,surgery time,and fracture healing time in the medial group were shorter and less than those in the lateral group(P<0.05).AOFAS scores of the medial group were higher than those of the lateral group(P<0.05).The incidence rates of postoperative complications in the two groups were close(P>0.05).Conclusion:Compared with posterior lateral approach,posteromedial approach can accelerate fracture healing,improve healing effect,and significantly improve ankle function.It is safe and reliable.
